5'8 120lbs. BWD 13.5 I'm a 32AA. 275cc moderate profile sizers look huge on me but hoping under the muscle will make appearance a bit smaller. My dr suggested the 290cc low profile but now I feel it is a little too flat and wide so no LOW's for me. My goal is about a B cup. I'm thinking 275 or 250cc mod. profile, but is this too narrow? Any bigger is just too much for what I want, based on sizers! Would 280cc LOW PLUS profile (width 12.5) work better for my goal? I don't want much side boob!

Despite your good description of body type and goals, online consultants will not be able to provide you with specific enough advice to be truly helpful. Ultimately, careful selection of plastic surgeon and careful communication of your goals (in my practice I prefer the use of goal pictures, direct examination/communication in front of a full-length mirror, in bra sizers, and computer imaging) as well as careful measurements (dimensional planning) will be critical. ***Given your concerns, it is definitely worth spending more time (preferably prior to the date of surgery) with your plastic surgeon to discuss further. It will be important for many reasons, that you feel comfortable that you and your plastic surgeon are on the "same page" prior to proceeding. Generally, I have found that very few patients benefit from the use of low-profile breast implants; most patients with your body type and goals benefit from the use of moderate or moderate plus profile silicone gel breast implants. Generally speaking, the best online advice I can give to ladies who are considering breast augmentation surgery (regarding breast implant size/profile selection) is: 1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. ***Ask to see lots of examples of his/her work. Sometimes, it is necessary to seek several consultations before you feel comfortable about your choice. 2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “very natural result” or "about a B cup” etc may mean different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on him who makes the bra; therefore, discussing desired cup size may also be inaccurate. Again, best not to discuss your goals and/or judge the outcome of the procedure performed based on achieving a specific cup size. The use of computer imaging may be very helpful during the communication process. The use of in bra sizers may also be helpful. In other words, use as many “visual aids” as possible during the communication process. Breast Augmentation is one of the most common procedures we do here at my practice. Every Plastic Surgeon has his or her own technique and preference of implant shape and size. Personally I prefer round high profile implants. For those looking for a more natural look without an obvious cleavage I would use moderate profile implants (less projected implants). Most of the time I like to place the implant under the muscle for many technical reasons. Usually the decision is made during consultation where I can physically examine my patient as well as getting a sense of my patient's desires and wishes. There are techniques to put the implants above the muscle or half way under the muscle or all the way under the muscle. Every patient's desires are different and every patient's chest characteristics are different. The more experience your plastic surgeon has the more likely you will have a successful and happy result. Answer: Implant size Unfortunately making a reasonable recommendation to you based on a photo with sizers in place and someone else's measurements is not really practical. This decision can be tricky under the best of circumstances. Measurements give us a good starting point, but I also like to provide simulation (Chrysalix now has an app through Allergan that will allow you to do this on your phone at home). Once that gets us an approximation, sizers can be helpful as well. None of these things is exactly the same as actually having the implant in, so an experienced surgeon is key.
