Question

I have seen very divisive opinions regarding alar rim thinning, is it actually possible but just a high risk procedure or is it actually impossible? If it can be done, is it possible to reduce a thick alar rim at 6mm by 2-3mm or is 1-2mm the absolute limit? Wha techniques could be used to do this? Thanks a lot (Green bar indicates the alar rim width Im referring to I want to thin this area)

In my experience, alar-rim narrowing can be performed in carefully selected patients, but it is not a simple “remove 2–3 mm” procedure. The single close-up photo shows the green measurement line, yet scale, angle, and lighting do not establish the true width or how much can safely be reduced. The fullness may involve skin, soft tissue, cartilage, or the alar base, so the approach requires an examination. Over-resection can cause asymmetry, alar retraction or notching, nostril narrowing, scarring, and breathing problems. Consult a rhinoplasty surgeon experienced in alar surgery and ask how the plan would protect both contour and airflow. There is no universal safe 1–2 mm limit; the achievable change depends on your anatomy and tissue support.
