Dr. Santdeep Paun is a Consultant Nasal and facial plastic surgeon. Dr. Paun earned his MBBS from the University of London before pursuing highly specialized fellowships. Dr Paun became a specialist on the GMC register in Otolaryngology - Head and Neck Surgery in the United Kingdom. He then completed a prestigious fellowship in Facial Plastic Surgery, during which he received advanced training at Amsterdam’s Academic Medical Clinic, Adamson Cosmetic Facial Surgery Clinic in Toronto, and Harvard’s prestigious Massachusetts Eye and Ear teaching hospital, respectively. Throughout more than 20 years in practice, Dr. Paun has frequently been called upon to lend his expertise in rhinoplasty to the press, as well as to future and current physicians. Q: 9 months post op, developing poly beak deformity? What can I do?

A: Thicker skin always takes longer to settle fully - the shrinkwrapping that occurs post op is slower and less complete. You have a little fullness in the supratip area rather than a real pollybeak. The first thing to consider is injection of steroid into this region which may well settle this down. Steroid injections are most effective sooner rather than later in the healing process so I would suggest seeking advice from your original surgeon about this as an option.

Q: Does bifid nasal tip get worse with age?

A: Bifidity of the tip cartilages does not necessarily change with age. There are some tip changes that can occur - usually a degree of drooping so there may be some related change. The tip drops as some of the support mechanisms that hold it up weaken with age and gravity also takes its toll!. There can also be some thinning of skin which may make the underlying tip cartilages look more visible.

So you may seem some difference to the tip but it wont specifically be more bifidity but the thinning of skin may perhaps make it look a little more prominent.

A: Hi

I don’t think there is a right or wrong in this question. It can be done either way although my preference is usually to do the rhinoplasty at the end. It reely depends what your own priorities are and whether the nose is bothering you enough to do it first. If you lose a lot of weight it can change the facial dimensions hence why I like to do the nose once everything else has settled so that the balance between the nose and the rest of the fact is harmonious.

A: Hi

The good news is that there is the potential to markedly improve the tip by elevating is upwards (rotating) and the securing it in the correct position. There are lots of ways to do this and it may involve attaching the tip onto a secure cartilage foundation to avoid it dropping again in the future. There is also a muscle going from the upper lip into the tip which can be divided to avoid the downward pull on the tip as you smile. This would in effect, lengthen the upper lip somewhat to enhance the overall appearance.

Q: My Nose Doesn't Look Any Different from Before my Rhinoplasty? (photo)

A: I think it is easy to see your concerns, but it is way to early to start worrying about the final appearance.  The swelling reduces rapidly in the first few weeks but then there is a much slower phase of healing as the skin shrink wraps itself.  Final results take at least a year but often more in thicker skin types.  Your surgeon should have regular follow up appointments booked for you so discuss your concerns when you next meet them, but give it some time for things to settle.
