Dr. Umang Mehta is a double board-certified facial plastic and reconstructive surgeon. At Mehta Plastic Surgery in the San Francisco Bay Area, he specializes in rhinoplasty, which makes up about 95% of the procedures in his practice. “What sets my approach to nasal surgery apart is that I’m continually trying to improve my techniques and get better as a surgeon,” he says. “Whether you took a hard fall on a surfboard and broke your nasal bones or want to straighten out a hump and lift a droopy tip, I can make your nose much more functional and help you achieve your aesthetic desires.” He is an expert at both first-time and revision rhinoplasty. Plus, he has extensive experience placing incisions both outside (the open surgical approach) and inside (the closed approach) the nose. Dr. Mehta’s plastic surgery philosophy is that patients should look natural and not like they’ve had work done. “For me, the goals are for the patient to look more refreshed and younger—to be a more refined and better version of themselves,” he says. To ensure he understands your aesthetic goals, Dr. Mehta uses computer software to “morph” your nose. Dr. Mehta received a Bachelor of Science in Biological Sciences from Stanford University (graduating with honors) and his medical degree from the Ohio State University College of Medicine. He completed an internship in general surgery and a residency in otolaryngology at Saint Louis University School of Medicine. He then completed a fellowship in facial plastic and reconstructive surgery at the world-renowned Lasky Clinic in Beverly Hills, California, and the Keck School of Medicine of the University of Southern California. Q: Rhinoplasty asymmetry due to scar tissue?

A: Thanks for the question. It looks like your bridge and tip are both deviated to the right. This is not due to scar tissue but rather your nasal bones and the top of the septum being off center. Straightening these out can certainly be achieved in a revision rhinoplasty. There are ways to reduce the swelling more quickly (steroid tablets, steroid shots) if you were to undergo another nasal surgery. If desired, your tip could also be made more refined and symmetrical at the same time. Kind regards, Dr. Mehta

A: An in-person exam would be helpful but it’s certainly possible that you may have some scar tissue under the skin that could respond very well to a steroid injection. I use Kenalog plus 5-FU for this. If your surgeon is unresponsive, you could seek a 2nd opinion. Kind regards, Dr. Mehta

A: Sorry to hear about your negative outcome from your rhinoplasties. It is absolutely possible to remove the cartilage fragments from your bridge and replace them with tiny diced cartilage pieces that are glued together with Tisseel. If you check out our Instagram feed, I recently posted this technique in a video and showed a case similar to yours. By dicing the cartilage finely and gluing it together, the bridge can be augmented in a smooth, natural, and predictable way. Rib, septal, and ear cartilage are all options. In fact, some of the cartilage that’s already there could be diced more finely and included. From the photo, it appears your tip could also be made more refined. A: Thanks for the question! I remove scar tissue and thin the skin envelope very frequently in both primary and revision rhinoplasty. This must be done very carefully to preserve the blood supply to the skin. I would have to examine you in person to let you know if you are a candidate... Sometimes if the cartilages were made too small inside the nose, there is empty space between the cartilage and the body fills this space with scar tissue. Often making a nose more defined in thicker skin patients involves actually making the cartilage stronger and taller, so it can push into the skin envelope from underneath. Other nonsurgical options include more aggressive steroid injections with 5-FU before and after surgery. Accutane therapy can also help. Q: Do Kenalog injections make that big of a difference in the final result? The surgeon refuses to administer them.

A: Great question. I use Kenalog injections often in my post-rhinoplasty patients. The injections are particularly crucial in patients with thick skin and those who have had prior rhinoplasties. The risk of skin indentation is low if performed correctly, and the indentations are generally subtle/temporary/reversible. The benefit outweighs the risk in my mind. Oral steroids can help too, but they don’t replace the benefit of Kenalog. I’d suggest seeking a second opinion, tbh. Pre- and postop Accutane therapy is another option for thick skinned patients.
