This 43 year old woman is 5'5 tall and weighs 133 lbs. She presented requesting removal of her implants. She had a prior breast lift when she had 225cc silicone gel implants placed below the muscle 5 years prior. Her implants/capsule are still very soft but she has developed some recurrent ptosis, (sagginess). She had removal of her implants and a partial capsulectomy with a repeat mastopexy, (lift), to have better access to the implants/capsule for removal as well as reshaping the breast. Her scars are still quite pink as the results are shown early, 10 weeks, after surgery.

This patient had breast implants for many years and over time the right implant developed a capsular contracture and both breasts had dropped. She did not want further implants and was against the scarring from an uplift. Therefore removal of the implants with capsulectomies was performed. After 6 months she has a very natural result and in fact the breasts look better. In these cases if you are not sure about the scars from an uplift, one approach is to remove the implants and wait and watch to see if you are happy with the result at 6 months. Often you will be surprised how much the skin retracts and you may be happy without a mastopexy. If you remain unhappy and wish to proceed to a mastopexy , this can always be performed at a later date.

Our patient visited from Hawaii and underwent a reverse mommy makeover. The reverse portion is the removal of the breast implants (explant) with a total capsule removal. Our patient is 1 week post op from Bilateral Breast Implant Removal (Explant) with Total Capsulectomy. She suffered from debilitating pain on her left breast/chest and shoulder region from the severely contracted implant. There was a significant intracapsular rupture of the left breast silicone implant with associated grade 4 capsular contracture. She is free of this heavy rock sitting on her chest and the dangers associated with a significantly ruptured silicone implant. Her shoulder and chest discomfort immediately resolved after the surgery.
