In your situation, I highly recommend considering a vertical mastopexy, commonly known as the lollipop technique. This approach allows us to create smaller areolas with minimal scarring. I prefer this method over the Benelli technique because it avoids puckering, nipple distortion, and undesirable scars that can sometimes occur with other surgeons. Particularly for cases with larger, puffy areolas and grade II or III nipple ptosis, the vertical mastopexy can yield excellent results with reduced scarring and improved nipple positioning.

Based on the photos you have provided, a breast lift, also known as mastopexy will be recommended by most board-certified plastic surgeons. This procedure is required to raise,reshape and tighten sagging breasts - to create a more youthful and lifted appearance.
I offer a specialized breast lift technique that not only lifts the breasts but also redistributes volume to the upper pole, resulting in a more augmented and sculpted appearance. The procedure involves removing excess fat and breast tissue from the lower breast and then using micro fat grafting to enhance the upper breast, creating increased cleavage and upper pole fullness. This approach is more durable compared to traditional skin removal/tightening methods, which may stretch out over time. Additionally, I reinforce the area under the most tension by overlapping the dermal collagen in the inferior breast.
There are optional techniques that I offer my patients to enhance breast shape/firmness and increase upper pole volume and longevity. One such method involves incorporating a small implant, not to increase breast size but to enhance breast shape. The small implant is positioned near the upper breast pole and this can improve the breast shape and firmness. The mastopexy is then performed to reduce and lift the nipple-areola, reduce lower pole volume and tighten the skin.
Another approach involves using Galaflex Mesh, a knitted surgical implant, to provide extra support during a breast lift. This FDA-approved implant is made of a biocompatible material that the body eventually safely resorbs. The mesh is placed internally to reinforce the breast structure, helping to maintain the lifted position and enhancing the longevity of the results.
Ultimately, the goal of my breast lift is to provide the patient with improved breast aesthetics and confidence, addressing the sagging concerns and achieving a more youthful appearance. I tailor the procedure to the patient's specific needs and preferences, ensuring a successful outcome.

Textured silicone implant vs smooth is there a much higher risk of bacterial infections with textured?

Numerous studies suggest that specific common bacteria tend to form a biofilm on the implant's surface, playing a significant role in the process. In general soft tissue infections are rare after rhinoplasty.
Sutures can sometimes cause irritation - so it is best that you see your plastic surgeon so he/she can evaluation your condition.

Before and after photographs would be very helpful to assess the degree of residual swelling. That being said, based on everything that was done during your rhinoplasty it would not be unusual to have upper lip swelling at five weeks.

Another lift with new implants & a tummy tuck, very skilled surgeon
I had a lift and implants done 20 years ago, then I had one more child 10 years ago. My breasts were saggy again and one of them was smaller than the other. I also exercised and lost weight but no matter what I did I couldn't get my stomach flat or strong. I was hurting my back all the time because of my weak core muscles. I finally decided to go in and get everything repaired. I had another lift with new implants and a tummy tuck. I can now say that Dr Reichner did a great job repairing what child birth and gravity destroy. I am so happy with my results. Dr Reichner is a very skilled surgeon.

I’ve seen Dr. Reichner twice. Once for my breast augmentation, 8 years ago, and the most recent, a tummy tuck. I’ve had nothing but an amazing experience throughout the entire process. Everyone I’ve interacted with, from my initial phone call to the one-year follow-up, has been exceptional and always happy to answer all of my questions and go over any concerns I had. Came out exactly how I wanted both the shape and size to look
It has been officially my 6 months post op since I have undergone my breast augmentation and I’m beyond happy with the results. I remember my first phone call to Dr. Reichner’s office and how sweet and welcoming the receptionist was. I scheduled my consultation and everyone were so kind and both the nurse and Dr. Reichner answered all my questions. The day of the surgery I was so anxious and the surgery staff including Dr. Reichner were so comforting and reassured me everything was going to go well. My results turned out exactly how I had imagined :). I felt no pain at all and was very patient with the healing process as the weeks went by. I can’t even begin to describe the overwhelming happiness I feel seeing how amazing my breasts turned out! The came out exactly how I wanted both the shape and size to look, and they feel so natural. I can definitely say I made a great choice choosing Dr. Reichner as the surgeon performing my breast augmentation. It is exactly 365 days since I had the procedure and I am completely thrilled with the results and how well they have healed. The shape is so natural, scars are very minimal, the healing process was speedy and they schedule you for many pre and post op appointments, red light treatments, vitamin regimens (optional but I feel they actually made a big difference). The staff were all always friendly & helpful. I definitely loved that I could text them any questions or concerns, and would get w text back with answers or get set up for an appointment if needed (and also that they would send me same-day appointment reminders via text, since I’m such a space cadet ^__^) Dr Reichner himself is very professional yet chill; his mellow laid back demeanor was def helpful at calming any pre surgery anxiety or post op questions.
