Dr. Christopher T. Maloney, Jr. is a Harvard-trained, board-certified plastic surgeon who returned to his hometown of Tucson, Arizona, and founded Maloney Plastic Surgery and Skin Care in 2002. Staff members who assist with in-office surgery performed under local anesthesia are trained in basic life support or advanced cardiac life support. Q: What is the average length of time a surgeon should honor a facelift price quote? Are Instagram surgeons driving pricing up?

A: Thank you for your question. Every surgeon has a different policy as to how long they will honor their quote. In my practice, we honor quotes for 6 months. It is common for fees to increase year over year. Each year a surgeon gains more and more experience, and often times someone who is really great at facial rejuvenation for example, will specialize in that procedure and begin limiting their practice to their "specialty". The more specialized, the surgeon becomes an "expert" and can demand a higher price for his or her time. The cost of practice overhead increases year over year for staffing, disposables, rent, utilities, etc. Most surgeons pick a dollar per hour they need to make in the operating room and increase that fee by 5-10% year over year. Facility fees and anesthesia often change year over year as well.

Q: If my BWD is 13.5, would a high profile 350cc implant be too narrow? I don't want to vary far from the 350ccs.

A: Thank you for your question. This is difficult to answer without the full story and the physical exam. This being said, you have a wide breast at 13.5cm. To fill the width, it would be nice to use a slightly wider implant. The moderate profile may fit the width a little better if you are wanting to stay at a 350cc. This said, the projection will decrease. I typically do not use a moderate for breast augmentation, but because of your width, you may want to use a slightly larger implant that is wider. You and your surgeon have all the necessary information to pick the best implant for you. Best of luck.

Q: Can you tell me pros and cons of mini neck lift vs full lift? (Photo)

A: Thank you for your question and for attaching photos. The best procedure for you in my opinion is a full neck/face lift. This procedure will tighten the neck, the jowls and give you a more youthful and refined jaw line. A "mini" lift is really just pulling skin and does not last. I tell patients a "mini" lift gives "mini" results. What you really need is the bands in your neck tightened and the underlying structures repositioned and lifted. Your surgeon can then tighten the skin and really improve your neck and jowls. Make sure to see a board certified plastic surgeon who specializes in facial rejuvenation and is an ASAPS member. Q: I have large breasts. Is augmenting possible? I'm a 38H in bra size

A: Thank you for your question. Without photos it is difficult to determine what options are best for you. If you are already an "H", adding an implant is in my opinion going to make you too large. The best operation for you to make your breasts more "perky" is probably a breast reduction along with a breast augmentation. I know this sounds strange at first, but the reduction will remove some of the excess breast tissue on the lateral (outer) portion of the breast, lift the nipple position and reduce the areola if needed. The placement of the implant, the augmentation will add volume and give you a nice shape. You should find a board certified plastic surgeon in your area who is an ASAPS member. Someone with Vectra 3D imaging would be helpful so you can see in 3D what a reduction alone vs a reduction with the implant will look like on your body. Best wishes. 

Q: What happen to the cheek fat pads with aging? Do they drop/sag? is it just volume loss? Both?

A: Thank you for your question. As we age the cheeks lose volume and begin to sag. The gold standard remains a face lift to reposition the underling structures to lift the midface, tighten the neck, improve the jowls and give you face a more youthful appearance. If you have lost volume, fat grafting can be performed at the same time as the face lift. You should find a board certified plastic surgeon in your area who specializes in facial rejuvenation.
