I regularly perform face and neck lifts on my Scottsdale-area patients. As we get older, we tend to develop deep wrinkles and sagging skin. Our practice is in Scottsdale and we do perform both surgeries under one anesthesia.  It is important that you select two specialists, one who performs a lot of rhinoplasty and one who performs a lot of breast surgery.  The skills involved in those two operations are very different.  having both surgeries at one time does not prolong recovery time and is a sensible decision.  I hope this answers your question.  I wish you all the best in your journey.

The best treatment for what you desire is called an augmentation rhinoplasty.  This involves using either cartilage or an implant to increase the size of your nose (on profile as well as the tip).  There are many pro's and con's of using cartilage or an implant. Additionally, if you choose cartilage, there are several options as to the source of that material.  It is important that you consider all options with your surgeon during consultation.

With respect to lip reduction, this can usually be performed at the same time as the rhinoplasty and usually involves incisions that are hidden inside the mouth.  Just as with the nose, examination would best help plan what is best for you to reach your goals.

Great question and thank you for your photos.  As I'm sure you could guess, the solution to the loose skin and muscle around your lower face and neck is to lift and tighten those tissues.  The procedure, done properly, should address your concerns.  Most notably, the jowls and neck will be helped the most.  Nasolabial folds sometimes require additional procedures or injectable fillers, but the vast majority of patients with your particular set of issues are happy after a properly done modified deep plane SMAS facelift.

The procedure as an easy recovery and most people are back to work and regular life within 10-14 days.

Don't be too concerned about your age.  If you have the laxity, like from the weight loss and/or genetic factors, the facelift is the gold standard treatment.


A complete examination during consultation would also give you the opportunity to ask more questions as well as provide you with additional options for your best outcome.

Thanks for your question.  In my experience, which hundreds of facelifts, drains are rarely needed.  I use a natural tissue adhesive and coagulation promoter at the time of the operation as well as a properly fitting dressing.  In the cases where the patient has more bleeding than usual (usually due to their genetics or other health factors) I will place small drains for 24 hours.  This happens about once every 18-24 months in my practice, and is well tolerated by the patients.  Although this is rare, no surgeon performing a proper facelift should guarantee that you will not need one.  After all, the goal is to keep your safety and the best results at the forefront.  I hopes this helps.  All the best to you!

Thank you for your question and photos.  As you guessed, a facelift would best address the jowls and jawline area.  This is because the cause of your issue is laxity and the best treatment is to lift and tighten laxity.  Less invasive options like using ultrasound or radio frequency ablation would be costly, painful and would almost certainly disappoint you.  Additionally, that particular area has some nerves that could be injured with those procedures.  

The good news about a facelift is that they really aren’t bad in your age group and, done properly, should last you a long time.  You can have a more detailed conversation in consultation, but done right, you will have a very natural result with well-hidden incisions that lasts for a very long time.  I hope this helps, and wish you all the best!

Thank you for your question.  Your photos appear to agree with your assessment - the tip is bulbous and the profile is tall and convex.  I also notice that the nose is wider at the base.  Based on the noses you like, I suggest that a rhinoplasty be performed with the goal of refining the tip, lowering the profile (side view), narrow the nose (especially the base) and raise or stabilize the tip a bit.  This would give you a very nice result.  The recovery, cost and details of that operation can be discussed during a consultation, which would include a complete exam of your nose.  I hope this helps you on your journey.  I wish you all the best!

Thanks for your question.  The good news is that deviated noses and asymmetric nostrils can be improved.  Of course, there is no substitute for an exam, but based on your photos, I believe your goal is reasonable.  A rhinoplasty would address the deviated nose and also bring some added refinement to the tip and overall width of the nose.  At the same time, nostril narrowing could be done to both reduce your nostrils and make them more even.  An experienced surgeon with an artistic eye would be your best selection.  Cost varies by experience, complexity of the nose and region, so it would be impossible for me to estimate that for you.  Find the surgeon you feel is right for you first, then discuss cost.  I suggest you never make such an important decision based on cost.  Revisions are far more complex and the cost can be significantly higher (not to mention having a nose you don’t like).  I hope this helps, I wish you all the best!

Hi shope,

Any surgery, even "just" a tip revision, will cause swelling in your nose that can take up to a year to resolve.  In many cases, much of the expected definition will be visible in the first 3-6 months, but it is not a reasonable expectation that it will not be swollen after a week.

Things you can do to help prevent this type of swelling are:  1) Consume a Low Sodium Diet  2) Sleep with your head elevated  3) Avoid sun or any injury (like squeezing blemishes)  4) Some surgeons advocate night time taping, however, those patients who are sensitive to adhesive would actually experience more swelling.

I usually advise against taping, however, some patients prefer it.

The short statement you have given is describing the harvesting of cartilage from deep inside the nose and taking care not to affect the support.  If that was all the surgeon performed, it should not cause a change in the tip rotation or columella at all.  

The posterior (back or deep) septum and superior (higher up) septum are fine to harvest, provided at least a 1cm wide "L" shaped part is still present along the bridge and down the columella.  

You really should discuss this with your surgeon to make sure there are no misunderstandings.

Thanks for your question and related photos.  Based on your short history and what I can see, I believe your nose can be improved upon. It is normal to feel that it may be hopeless after you have gone through more than one surgery and did not get the results you desire.  However, in the hands of a surgeon who does a lot of complex secondary (revision) rhinoplasty, your chances are very good.  Because I do a lot of this type of corrective work, I am comfortable with your nose.

You do appear to have a complex columella and tip problem, which would likely require grafting of septal, ear or rib cartilage (depending on what is available) as well as making the aesthetic changes you desire.  Keep in mind that there is no substitute for an in person examination and discussion.

Top 1% of facial surgeons - I finally see myself again

Dr Holden is Hands down the best facial surgeon in the Western Hemisphere. I’m not even close to exaggerating. My botched story happen 15 years ago in a Chandler Arizona. Without going into a full blown harrowing story of my life let’s just say my Botched experience was so bad that I quit my career job because I was a B2B client facing sales executive. I lost all my friends because I would not go out anymore and even contemplated suicide. Life was over for me. Making no money I had started a new career where I could hide behind a phone. At that time starting 15 years ago I began making and going to as many Facial Surgeons as I could…I hit almost every Facial surgeon between Arizona and California. I bet it was close to a 1000 surgeons over the years and even found and hired one in Newport CA. that was well known around the world for his craft. Long and short of this part of my story is that not even the DR that works on half of Hollywood could help me. He tried but he just couldn’t do it. I had a great experience and paid a ton of money but still the problem was not taken care of. Okay so at this point of my experience let me tell you a little more detail of this Facial procedure that happen to me. The Botch was easy. That Doc had injected who knows how many cc’s of a permanent product called Artifill into the sides of my chin. Artifill is basically cement. I had cement injected onto my chin bone making me look like I had somehow stuffed a golfballs under my skin on each side of my chin. It was horrible. So now you can envision the problem I had. I looked like a totally different person. Let’s go to now it’s 2020 and I am still on the hunt for a Facial Surgeon that could remove this product. Amongst all the issues surrounding this Artifill product the feedback I had learned over the years was it will be impossible to remove not only because it is permanent but it is also attached to a main nerve in my face that helps produce saliva. That alone scared off 90% of the Surgeons and the 9% that thought they could do it I didn’t trust…I didn’t get that gut feeling. 1% - Dr Paul Holden…….I somehow networked my way to him. God definitely put me in contact with him. After multiple consultations I made the appointment. Today is 2 months after and I’m literally crying while writing this. Looking in the mirror I finally see myself again. I feel that the Botch Doc stole 15 years of my life. I am literally out of words. By the grace of God I met Dr Holden and I can assure anyone reading this needs to look no further and just make the appointment. Dr Holden is the 1% the best Facial surgeon I can honestly say that with facts and proof. One more note to his entire team is “Thank you all”. A reasonable price for a good plastic surgeon with a caveat

Let me start by saying that the price was very reasonable and that Dr. Holden comes highly recommended. His practice is what we lawyers would call a "mill." Get em in, get em out. After my initial consult, I did not see the doctor until nearly a year later due to the wait list. I thought I was getting a neck lift which I later learned was a FL. That said, I was not prepared mentally or emotionally for a major surgery like this. He saw me the day following the surgery and gave me flowers which was a nice touch. I was still really out of it from the anesthesia. Exactly two days later (Friday) I did reach out to Dr. Holden who said that he had not cut into my face (more on this in a second.) My face was so distorted and I was in a state of shock or trauma and did not know it. The next day (Saturday) I called Dr. Holden because I felt like my windpipe was closing because my neck was so tight. He reassured me everything was fine and not to worry. I did feel reassured after that call but did not know at the time that I was experiencing swelling and hardening of the tissues, also normal post-op, but it was something I didn't understand. 2 weeks later my left ear began throbbing. I did call to speak to one of the girls that works with Dr. Holden, and it took three days to get a call back! Dr. Holden did call me back but never once checked on me about my left ear. I thought that was rather odd. I did see him every other week post op, but anyone who has been through a major surgery knows that the big questions come up when you are not in the office. I saw him yesterday and said I was for the most part happy but that I did feel a "don't call me, I'll call you," vibe. He said that I was hallucinating and that he wasn't my therapist and that he had spoke to me "a lot." I don't know if you have been following along, but there were THREE PHONE CALLS. That is not "a lot' following a major surgery wherein you don't know what is happening with your body. The hardening of the face was the scariest part for me and I still have quite a bit of it on my face and under my chin. I did research on the internet and found that not only do plastic surgeons give out their cell number for patients suffering from post op issues, but they have "an open door policy." For example: Joe Niam @lovethatface.com. I understand the perils of giving out my cell phone number and have had clients call and text up to 50 times a day!! So believe me, I understand. But 3 calls in just over a month is not a lot, especially when one of the calls had to do with pain to the left ear (still not out of the woods on that one.) I was sorry that we were unable to resolve the breakdown in communication and most of my caregivers are in Los Angeles and have been in my life for over 20 years. I do not abuse the use of the phone, but telling someone they are hallucinating two days out of a 7 hour surgery was just wrong. To get back to the beginning of the story, my face appeared rather distorted when I went to change the bandaids. Even my cat didn't recognize me! I really have no other complaints other than I still have left ear pain and emotionally I feel like I was gaslighted for just being a regular patient. Speaking to him about these post-op issues really made me feel much worse. I still don't know if I am healing correctly, I am just going by my intuition. I was there yesterday and nobody said anything about my appearance. Just know going in, you will be flying alone pre-op and post-op and try to have someone with you for the first week. I did have my sister there but she didn't understand what was going on with me, just like my therapist would not known what to do---he would have said "call your plastic surgeon!" Dropping by to share some encouragement for those still in the planning process. This procedure is something I've wanted for a long time. I was tired of feeling aged and exhausted in appearance wile I was thriving! I Finally found a doctor I felt comfortable enough with to take the plunge. If There are other African-American potential clients out there who just want to feel comfortable knowing their doctor understand their specific needs I too need to put myself out there and be vulnerable enough to share my journey. Here you will see before through day five! I am so excited to see my final results! Updated on 25 May 2022: Day 7 plenty of darkness but not as much puff! eyes feel wider and more alert today. Going to get my stitches out tomorrow. Sidenote I was a smoker before I was advised to quit in order to get this surgery by my surgeon. I don't know if that plays into it at all since I remember reading a lot a previous smokers take a longer time to heal and could have more darkness longer’ AnyWho I’m feeling great and I’m excited on how these results are headed Updated on 13 Jun 2022: Pardon the lipstick! Almost 4 week update! Still have darkness under the eyes but fading every day! Still super happy with the progress and my eyes feel so much more alert and refreshed! I wanted Rhinoplasty for a pretty long time, but was always scared, or on the fence about it because I have seen so many bad procedures. Bad as in overdone, or a nose with 2 holes and tiny point that stood out as "a nose job." This was my fear. I did not want to look so different that people would wonder what I did to my face. I explained all my concerns to Dr. Holden. He assured me this would not happen, and made me feel comfortable with the procedure. It has only been 18 days since my procedure and I'm very happy with the changes. It is subtle, not drastic, and I still look like myself. I went back to work after 1 week and no one really noticed I even had this done. One guy asked me if I changed my hair. He thought something looked a little different, but could not tell what. I think this is a sign of a good procedure. Cosmetic surgery should look natural, and not overdone. I would highly recommend Dr. Holden if you are considering Rhinoplasty.
