Its hard to say without a physical exam. Its possible the lacrimal gland wasn't fully repositioned or that the dislocation has reoccured. Your doctor may want to get imaging like a CT scan, or even potentially revise the surgery. I'd make sure you consult with an oculofacial plastic surgeon before considering a surgery like this in the future.

Blepharoplasty surgery can change the volume and sagging issues that tend to occur as we age. However, many times patients also complain of wrinkles or dark circles. When the area of concern under your eyes goes down to the cheek there is often a fluid component to the problem. You may want to consider non-invasive treatments like radiofrequency microneedling. Surgery with a blepharoplasty may be able to help, but often in a scenario like this the surgery causes inflammation which may make the under eye area look worse before it looks better.

I would go with the opinion of your oculofacial plastic surgeon. Most plastic surgeons work on the entire body, from head to toe. On the other hand, oculofacial plastic surgeons sub-specialize in complex eyelid and facial plastic surgeries. It looks like you have excess skin that your oculoplastic surgeon can help you with.

Upper Bleph + Under Eye Filler? (Photo)

We have many patients who successfully undergo upper blepharoplasty despite having skin of color. There are several things that can be done to minimize the scar after surgery, including the use of silicone gel after the initial wound heals. We have patients start this after around 2 weeks when scarring is a concern. Also, the upper blepharoplasty incision is made in the eyelid crease, so this is generally easy to hide when the surgery is done well. Regarding lower eyelid filler, it is a topic which is pretty controversial. In my experience, it is only around 10% of patients who are good candidates for under eye filler. The vast majority of patients I see would have preferred if they were never treated with under eye filler.

It is difficult to say exactly what you would benefit from without a detailed consultation with eyelid measurements. True ptosis is when the upper eyelid margin sits within 2 mm of the pupil at rest. The best option is to get a detailed evaluation by an oculofacial plastic surgeon who can advise you best on options in your specific case.

The question about what exact surgery will help make the eyes look more open in a natural way comes up frequently in my practice. The short answer is that a detailed history, physical exam, and preoperative photos are needed to make a true assessment and give you good options for surgery. You'll need to weigh the risks and benefits of each option. In my practice most patients with your eyelid position will be quite happy after an upper blepharoplasty with browpexy surgery. That said, the examination may reveal underlying brow ptosis or upper eyelid position asymmetry which is best addressed at the time of upper blepharoplasty. Eyelid surgery - The procedure was a walk in the park

First and foremost, I could not be happier with my results. Hands down… One of the best decisions I’ve ever made was working with this team. Dr. Phelps is truly talented at what he does. A true artist! I am still healing, but, I am already over joyed with what I am seeing. I’m looking more and more like myself every day after years of trying everything to correct damaged under eye skin. Dr. Phelps erased years off my face, I don’t even have an expression wrinkle when I smile now. For years I avoided this procedure at all costs because I was nervous about it. The morning of the surgery, my anxiety got the best of me and well… let’s just say I had a mini panic attack…. Not my finest moment. Dr. Phelps and the whole staff stepped up and were the exact support and reassurance I needed to follow through with something I wanted so bad. They calmed my nerves and put me at ease. I cannot be more grateful for the compassion I felt. The procedure was a walk in the park. It was so quick (1 hour), easy and painless! I was out and at home and felt normal that same afternoon.
