I believe you are absolutely right in prioritizing your back over your breasts, at least for the moment. In general, a reasonably sized implant probably will not cause or significantly aggravate your back issues, and the types of breasts that really cause the most back issues are large droopy breasts, because they significantly change your center of gravity and pull your upper back and neck forward more than breasts that are not as droopy. That being said, you really need to discuss this with your spine surgeon and perhaps also a physical therapist. If they do give you a go-ahead to proceed with surgery, I would recommend agains going too large. Good luck!

It seems unlikely that a breast augmentation of a moderate size will cause further problems with your back. A heavy or large breast can cause upper back strain when a support bra is required, though lower back issues and herniated discs are not commonly caused by even the heaviest breast.
