Lower eyelid surgery (Blepharoplasty) is one of the more difficult operations. I have seen the various techniques achieve similar results.Therefore, as long as your surgeon has made the correct evaluation of your problems and provides the proper operation to correct these problems, you should get a good result.
Selecting your surgeon is important; your surgeon should be have a conservative and effective technique for treating the lower eyelid. The general trend in lower blepharoplasty is toward conservation of the lower eyelid skin, preservation of lower orbital fat, avoiding tension on the lower lid, performing a canthopexy or canthoplasty and filling the tear-trough depression.
I have been doing more and more Botox, laser resurfacing or minimal excision of the lower lid skin and correction of the tear trough with fat or a hyaluronic filler on women in their 40's and 50's instead of blepharoplasty. This is also a trend seen with brow and forehead lifting -- the number of operations has dramatically dropped off with the widespread use of Botox.
Selecting the right plastic surgeon is very important with this operation. Most plastic surgeons will admit that lower eyelid surgery has greater risks that upper eyelid surgery. Remember, over resection of the inferior orbital fat can lead to a hollow, sunken appearance over time and should be avoided.
