Transconjunctival lower blepharoplasty with conservative fat removal with or without appropriate fat repositioning is often the procedure of choice if significant deformity is present.
For minimal pseudohernias, a procedure called transcutaneous fat cauterization using a # 22 or 23 IV catheter inserted into the fat pocket under local anesthesia can be done by an experienced surgeon safely.
The fat is cauterized using low power settings and usually no pain is felt. There is some swelling and there can be bruising, but usually it is minimal. You must ask your surgeon if he is experienced with this technique before agreeing to have it done.
