Younger breasts with more dense breast tissue will stay fuller after breast reduction. The more mature breast with more fatty tissue in it will tend to be a little more flat after surgery. Also, if you have had children and breast fed, the risk of loose skin goes up. Even if you are on the higher risk side of the equation, fullness can be returned with the use of a small implant after lifting the breast if it becomes necessary in the future. This, however, is a far from common approach.
You want a look that is natural and that fits your body. It is important to discuss both your functional and cosmetic goals during your consultation. If you are getting your health insurance to pay for your breast reduction, then there will be a minimum amount of tissue that has to be removed and this could alter how your breast is optimally shaped.
