Answer: Implant Exchange with Ultimate Breast Lift(TM) Your 800 cc implants are too great of a mechanical load for your body to maintain. You have already had previous attempt at lifting using a vertical scar technique. At this point, you need your implants removed and a simultaneous lift. I recommend a new technique called The Ultimate Breast LiftTM. This technique will reshape your breast tissue creating upper pole fullness, elevate them higher on the chest wall and more medial to increase your cleavage. At the same time, implants can be re-placed and your areola size adjusted. This technique will eliminate your ugly vertical scar and maintain your nipple sensation. I always recommend small round textured silicone gel implants placed retro-pectoral since they look and feel more natural, are more stable, less likely to ripple or have complications needing revision. Implant profile is irrelevant in the retro-pectoral position since the muscle compresses it. Best Wishes,Gary Horndeski, M.D.

Answer: Implants are too large Dear Renee, It appears from the photos that you have already had a lollipop breast lift. Regardless, your implants are too heavy for your tissues to support. I would recommend downsizing the implants and redoing the lift. Good luck!

Answer: Internal bra and downsizing implants for bottoming out Large implants such as yours often cause bottoming out because they are too heavy for your own tissue to support. Going smaller makes sense and because the skin has been stretched a lift will likely be needed too. Even with this however the use of an internal bra with a material such as Seriscaffold can be very helpful for adding support and coverage. Strattice is another option.

Answer: Decrease implant volume and revise lift Thanks for your question and pics. I would guess that the picture you posted as your goal is someone who is young with good elasticity. They appear to have high profile saline implants that are likely encapsulated and hard. Looking at your picture, the large size has so much weight that it has exacerbated the asymmetry of your nipple position. My suggestion would be to significantly reduce the volume of the implants and then go back later and revise the mastopexy. I think your long term and short term results will be much better. Best of Luck!
