What are the most common scenarios for eyelid surgery to be deemed medically necessary?
Some of the criteria that insurance companies are looking for are: excessive skin weighing down the eyelids, patients having to look at their eyelids skin and/or eyelashes in primary gaze. Recurrent skin irritation from excessive upper eyelid skin. Marginal reflex distance; the distance from the light reflex from the camera to the lower limit of the upper eyelid margin and their results on visual field testing.
When some fat compartments in the lower lid have atrophied with age, but other compartments remain and therefore appear herniated, what is the best surgical or non-surgical treatment to address this? Remove the herniated portion or tighten the lid? Is it wiser to seek an experienced eyelid injector that can add filler to hollow areas to "level out the playing field" instead? (if filler in this part of the orbit is even an option at all). Sorry for so many questions!
You may want to approach the situation with a transconjunctival removal of the protruding fat pads of the lower eyelid, with an additional skin pinch removal of skin just below the eyelashes. In addition, I would also recommend that you have either a hyaluronic acid filler (Restylane) or fat injections done to the hollows below the eyelids (tear trough region). This way you improve the eyelid contour without creating or exacerbating the hollow appearance.
Candidate For Transconjunctival Blepharoplasty or Restylane Filler? I am 24 years old and suffer from under-eye puffiness. I'm in dire need to correct this situation in order to restore my youth and happiness and not hide behind my glasses as I've been doing for years. Am I candidate for the less evasive Restylane filler? Or would you recommend full Transconjunctival Blepharoplasty? or im to young for this ?
You are actually a good candidate for several procedures. Certainly injections with Restylane to the tear trough area will improve things. If you are looking for a long term solution, you may want to consider a transconjunctival approach to some fat removal with the addition of fat injections to the tear trough region. Another solution would be fat repositioning of the protruding fat in lower eyelids over the orbital rim, this would also improve the tear trough depression.
