While there aren't many breast implant reduction posts and very few relating to small implant sizes, all of your stories have been have been helpful and illuminating. Thank you!

I am 53 years old, 5'4, 120 pounds, 35.5" at breast center, 32.5" at bra bottom, 27" waist, 34" hips. My first and only BA was 20 years ago - saline implants over the muscle but don't know the cc size. I was a 32AA before and am now a 32D. I've always thought them too big but have mostly loved my implants, have thankfully had no issues and except for sagging (aging!), and they're still in good shape. But I'm ready for something smaller - not explant small but definitely smaller by half at least. I want them to be less conspicuous and am ready for more clothing options.

I had a consultation four years ago with a highly lauded surgeon who recommended I think it through more and maybe wait until I had an issue because they're still in such great shape and going to smaller implant would require a lift causing vertical scars. I thought that was good advice at the time and did wait until this year when I decided it was worth it. I am ready to be smaller! I went back to him for another consultation and he was a total jerk, lecturing - actually bordering on shaming - me for coming back and even asked what my partner thought. (Why is that important?!) His assistant literally slunk out of the exam room during our "discussion".

My next consultation was the exact opposite. Dr. Derby was happy to explore all the options and possibilities. We spent a great deal of time talking about what it would take, the challenges, what she thought would work, the different implants now available and the crazy disclaimer paperwork. She said I'd get a call the next day with pricing and scheduling possibilities which I did. Due to some travel, Judy (her RN) was flexible regarding their pre-op testing timing and I've now completed those requirements and am scheduled for my surgery on May 26.

Because I don't know what size my implants are, the new size is a guessing game. I'm childless and post menopause and as far as she could tell through her initial exam, I still have no or very little breast tissue and she guessed my implant size at 350-ish cc's. I'd like to be half the size I am now at most. She conveyed one of the potential challenges is to re-adjust my skin appropriately so the implants fit snugly and everything ends up in the right place aesthetically. (My explanation - I'm sure she was more eloquent and technical.) :) As I understand it, going under the muscle this time will help a little but a judgement call may be needed during surgery regarding size to get the best overall results. In my phone conversation with Judy, she said they'd probably order a couple sizes because they don't do many small implants.

I'm going to check in with them this week with a couple questions ... Would it be helpful to deflate my current implants at my pre-op appointment to get a better idea of size? (10 days pre-surgery -- I can live with stuffing my bra for 10 days, right?) What brands and profile options do they recommend? In our consultation, she thought round and smooth would probably be the best for my body but my head was spinning a bit at the options. I don't remember anything like these choices 20 years ago!

Any thoughts you have are welcome. I'm more excited than nervous at this point but I know the nervousness will definitely come. Thanks again, everyone, for sharing your stories. You are all amazing!

Hi Sharon, I too am having an explant and going smaller. I have consulted with 3 different surgeons and have another appointment with a PS on May 13th. I have 700 ccs over the muscle saline placed in 2012. No lift at the time though In should have had one since I had been a breast feeding Mom. Pre implant I was a 36 C but lacked fullness and pettiness. Even at 5ft 9in and 150lbs. 700 ccs is way go big. I now wear a 36 FF to 36 G depending on the bra. I look made only and the implants are so heavy I droop even more than I did pre-implant. I look forward to your progress and updates. Thanks again for sharing.:)

Hi Janelle ... it's so crazy, isn't it, the sizing part?? Are you going to replace your implants or just remove them and get a lift? I'd imagine you feel your FF/G's as much or more than I do my D's -- you're a lot taller but still only 30 pounds more. In the past several years as I've become more self-conscious of them (maybe a growing older thing), I've even noticed I've started to hunch my shoulders so they aren't so prominent - the last thing we need to be in the habit of! I'll follow your progress and wish you all the best. Thanks for reaching out. I know we're gonna feel a lot better when it's done!

Yes the weight is awful. I have permanent bra strap marks on my shoulders and my neck, back and shoulders always ache. I am vacillating back and forth between having the implants removed and hope I have enough natural breast tissue to reshape and a full lift to make them perky and pro use some upperfulness or go ahead and have an implant exchange to much smaller with pocket revision, internal bra and a lift.

I had my first BA 10 years ago and let the doctor talk me into the size. I got 375cc gels under the muscle. I went from a 34 barely a B cup to a 34 DD. I was so uncomfortable with them, but the doctor assured me I would be sorry if I went smaller. Well I smashed them, covered them and finally replaced them! Fortunately my new doctor was in agreement that my 5'5" 120lb self did not look natural with DD's. I let him make the decision on size in the operating room and I'm so happy! I did not need a lift just a new pair of 275cc gels under the muscle in the same pocket. I am now a 34 C and I feel like I always wanted to. My breasts are full but natural. That was my goal in the beginning, full natural breasts. So glad I did it!

Same here. Same thing I was told. This new doc is so understanding. I cannot believe these gals getting 5,6 or 700cc. I hope they are thinking if the future not just the now

I too am going smaller. I got talked into going large 18 years ago. I have never been happy but cost stop me till now. I am 59. I found a great doctor. He said since j have saline he deflates then a month later will decide what I want. I get deflation on Tuesday and after the pics j have seen an nervous but cannot wait to be more comfortable and not have to buy large shirts so I can button them. I was a 34a ended up a large 36 d. With age I am now a 38 d

I had a consult with a highly recommended PS in Seattle. He will deflate saline implant 2 weeks before explant and full lift. Like you I am looking forward to a smaller me.:) if after the full lift I don't feel full enough I will go with a small under the muscle implant. I was warned by the PS that after deflation my breasts will look a little sad, but the final result will be better if I deflate even a couple weeks prior to the lift procedure .

Had deflation a few hours ago. Still lots to have done I already feel so much more comfortable Surgery not till end of June due to getting time off, but gives skin time to reduce a little. Will be going with silicone after talking to doc today

I almost went with deflation and waiting but it's pretty obvious I still have so little breast tissue, it just didn't seem worth it to me or the doctor. When is your explant, Dee? Look forward to updates from you both, Dee and Janelle.

Deflation was Tuesday. I felt better as soon as I walked out. I has a good amount of tissue and good skin doc said. My surgery is not till June 27 as I needed to get time off. I will have 12 days off. Hope it is enough. I had so much pain and bruising when they were put in. I think because they were too big. He pulled out 425. I will still get implant but much smaller as I originally wanted

Fabulous! I know it's hard to plan for the long recovery but I'm a believer that taking care of yourself well after a surgery is almost as important as the procedure itself. 425s. Wow. I'm glad you're feeling better!

Getting an explant and lift. I saw the new PS yesterday and he couldn't believe that the prior PS for the first BA thought 700ccs would look good on my frame. He said my shear and rib cage were too small to carry that size and he could imagine the severe pain I had been living with.

Hi Sharon - I'm 52 years old -5'5"and 115 pounds. I had salute above the muscle 24 years ago - finally re-did and wanted to be smaller like you describe. Sounds like you already have had thorough meaningful discussions-which is great bc it isn't nearly as easy to get a great outcome - from my experience....
I ended up with 2 surgeries , 2 doctors... When he was in HE decided to put the same size in bc he said smaller didn't fill me out-it was awful- big and hung down over my rib age - I looked matronly !!!! Not perky and youthful . I needed a lift to go smaller and get an aesthetic desire able result - so much better-but had to live with a terrible job that I spent $7,500 on for 4 months to heal before I could have a revision ...

Hi Robin ... Oh, I'm so sorry! And I so appreciate your comment. This site has really helped me think it thru but my consultation with the Dr. who's doing my revision really helped, too -- she took the discussion to placed I hadn't really though about. I decided I need women doctors from now on. Are you happy with them now? How long ago was your revision? It really is interesting, isn't it? Sharon

Hi everyone ... I had my surgery a week ago yesterday and am finally at a point to post an update. I went in at 7am and was out at 1-ish with the surgery itself lasting about 3-1/2 hours. Everything went as planned and they were terrific. My first couple days were a blur of muscle relaxers and pain killers to help with really intense pain -- 9-10 on a scale of 10. Yes, this type of revision is not for the faint of heart ... it hurts!!

Day 3 and 4 were a bit better but the pain still pretty up there at 7+. Thankfully it started to taper back a bit after that and by day 7 I'd say I was at a 6. I started backing off significantly on the pain killers (my choice) but was and still am on muscle relaxers every six hours.

Today is day 8 and I'm sitting at a 5 on the pain scale but it's better. It now feels more like what my original surgery (above muscle saline) felt like - super tight, sharp nerve pain bouncing around randomly - but with the added component of the muscle spasms. Whoa. They told me about them but there's really no getting it until you feel them. It's hard to get a deep breath because of the tightness, and heaven forbid you sneeze or cough - ouch!

A major problem that I wasn't as prepared for as I should've been is the constipation from the anesthesia and pain meds. It's the major reason I've nearly eliminated them -- it got extremely painful and uncomfortable. I started stool softeners the day of the surgery but should've maybe started in advance. I went to triple doses of laxatives by day 5 and finally got some relief starting day 6. It was nearly as bad as the surgery pain. I don't normally have any digestion issues so this was new for me. If I had it to do over, I'd start on the stool softener at least a day before the surgery and start on laxatives jut after the surgery or the next day for sure. I recommend eating only foods like grapefruit and other citrus fruits, bran cereal and vegetables. Popcorn has been a good snack for me and at least kept me in some butter. :)

Relative to how they came out, I'm really pleased! Going from large over the muscle to smaller (210cc) under required quite a bit of work as you can see. The white is just the stere-strip on stop of the cuts. There are no stitches to come out and the strips will fall off on their own. They have a long way to go before the swelling goes down and they drop and fluff and do all that stuff but I feel I'm going to like them when I'm done healing. I have a tendency to push myself too hard and the nurse warned me yesterday that I really need to continue to take it easy and look at this as a 6 week marathon vs 2 weeks print. I'll try.

I have my post op on Thursday and will post more photos then. One thing I've really like about my doctor is that I don't go back for post-op for two weeks which makes sense to me now that I've been through it -- there's just no need unless there's a problem and I should be resting. I also got to shower starting the first day after surgery. My partner was great helping me at first -- he didn't think it exactly rough duty! :) I'm on my own now which is a bit tough but fine. No arms above the head so the hair is au-natural and it's actually pretty freeing.

How many cc were you and what did you go down to? I'm having the same in June 27. Can't wait. I had saline under Muscle for 18 years. Always felt too big too uncomfortable Don't want to be flat do I'm thinking about half of what I had. Will see in preop

I had 350cc above the muscle and went to 210cc Natrelle moderate profile under the muscle. Good luck to you!

You look great! I'm looking into doing the exact same thing. Your previous breast look a lot like mine. I have 400 cc and I cannot wait to take them out. I'm looking into 200-250 the most. Thank you for sharing!

Thanks! They're still a little different in size, but that should work itself out over time. Even now they feel so much more "sporty" if that makes sense. :)
