I am 31 years old. 5'7", 130lbs, 10% body fat (very little natural breast tissue). In March, I went in for an implant exchange. I had saline high profile 420cc (approximately) submuscular. I was unhappy with my shape, and also wanted to go a bit bigger. The shape I was looking to have was to have more side boob. I truly hated the way my breasts looked from the side. My original implants were done well; I had no CC, no bottoming out, no lopsided or unevenness, a tiny bit of rippling (which is to be expected with saline, I am told). The cleavage was fine, they didn't touch; there was about a two finger space between them, but I was fine with that. Though they looked "fake", they didn't look like round balls attached to my ribcage.
So when I went in for my exchange with the following requests: bigger size, more side boob, and I was satisfied with my cleavage.
My new doctor suggested going with high profile saline again since I had good luck with them the first time. He said since I wanted a larger size, the width would be bigger anyways. We decided on 700cc. My current implants are mentor high profile saline 630 filled to 700cc submuscular. It's been 3 months and I've hated them everyday. Again, I've had no complications whatsoever. But I hate the placement and shape. Instead of the implants creating more fullness on the outsides like I wanted, I now have all the fullness in the cleavage area. If I look down at my cleavage, it looks like they are three inches thick on the cleavage side and about an inch thick on the outerboob side. I now have virtually no hourglass silhouette shape. My silhouette looks straight around my ribcage, almost like I have no boobs at all. But when I turn to the side, there they are!
The doctor who performed the surgery is no longer with the practice, so I've been discussing a revision with another dr in the practice. He is suggesting sientra cohesive gel textured shaped oval base in the 700cc to achieve the width and projection I desire. I'm a little hesitant since I'm having trouble finding before and afters with an oval implant. I'm just so unsure of what to do at this point, all I know is that I am so unhappy with what I have right now. Any suggestions or help would be greatly appreciated!

I believe that Dr. Pousti uses a 800CC Moderate Plus implant which he overfills. Mentor Catalog number 350-2800. http://www.mentorwwllc.com/global-us/Breast.aspx. I am not an expert, but I would suggest measuring how wide you want the implants to be from your armpit to your cleavage, leaving at least an inch gap in the middle. Another way you could approximate would be to find out what implant you have now and look up the measurements and then measure the distance between where the boundary of the implant is now on the side of the armpit and where you would like it to be. Then look for an implant that much wider than your current implant. I think the problem would be resolved by swapping the high profile implant for a moderate profile implant with more cc's. The whole purpose of a high profile implant is exaggerated forward projection. Those high profile implants actually sacrifice width for additional forward projection. If you can, go see Dr. Pousti.

I completely agree. I just had my revision a week ago and when I met with my ps he told me that having a tear drop implant, the oval shape if i'm not mistaken, will have more chances of rotating because you already have the round implants. For the past few months I've been looking at hundreds of pics and I notice that most girls that have very little breast tissue like me get a wider shape with cc'c that end up with 00. Like 300, 400, 500. They also look more natural with silicone. Have a look at my pics, I got 500cc silicone. I hope that helps. Good luck

I understand not being satisfied with results especially after spending so much money. If you wanted side boobs you needed to get moderate or moderate plus profile instead of High Profile implants. I suggest you go to a really good PS in revisions. I heard Dr Pousti is one of the best. Best wishes

I used Dr. Pousti for my last surgery and he is the absolute best. He filled my implants to 1780 cc's.

What are you thinking of doing size wise?

I'm sorry that you are unhappy with your implant surgery. In terms of recommendations, I don't recommend the shaped Impants for you. I don't believe they will give you the breast shape and size you are looking for. Based in your description I believe you are looking for the nice rounded shaped breast but wanting the overall width to be wider. This can be done with a high profile round smooth silicone implant. Because you already have 700 cc of volume, you can easily change out the implant with either 700 or 750cc due to the fact that the pocket is already made. By using a high profile silicone implant you will reduce the projection at your cleavage about 1 cm, but will add about 1-1.5 cm to the diameter. The high Profile saline implant will only make your breast look more rounded and more narrow and give you more projection at the cleavage point. I hope this is helpful. I recommend getting another opinion from a board certified plastic surgeon in your area. If he has experience with breast augmentation with various implant styles he should have plenty of before and afters for you to see.

I'm having surgery June 24th and am going back and forth between 650-700cc silicone gel ... My friends are saying I'm crazy and my boobs will be huge but when I tried on the sizers I liked the amount of side boob these gave me. What made you go from 700cc to an 800cc instead?

Honestly it was the Drs suggestion to go to that size to achieve what I wanted. Going from 700cc saline to 800cc memory gel (both were HP) provided a wider diameter, less projection and more of the weight in the bottom rather than at the cleavage. These were all of the things I was looking for. Also the difference from going from 700 to 800, I didn't feel like it would make a *huge* difference in size so I wasn't opposed to going larger.

Do you have pics of the 800cc? Do the silicone fill much heavier than the saline? Thanks!

Yes! I have pics. I wrote a separate review for Dr. Revis who did my 800s. As for the weight, I don't notice much difference from the 700cc to 800. Although, the first few days, they felt "weird" to me. I think because it was a different material and they hung differently and moved differently. But after like 2-3 days, they felt like my own. I think I remember Dr. Revis saying that actually the memory gel weighs less than saline.
