I was diagnosed with extensive BC in the right breast and opted for skin-sparing (and left areola and nipple sparing) bilateral mastectomy, with immediate reconstruction (path showed DCIS, LCIS, ILC and ICC, Stage IIb N1aMX, ER+/PR+/Her-). I wanted to have same size of breasts, and the entire procedure to not be longer than 5 months, as my insurance is ending on April 2010 (reconstruction is fully covered).

The plastic surgeon I have seen recommended double latissimus dorsi flap with direct implants. I have told him at the first visit that I am a smoker and due to the stress caused by the recent diagnostic I am not sure if I can quit smoking. I was told that the procedure will require about 6 weeks of recovery, and will bother me only if I would want to do professional tennis or swimming.

To make things short - I lost the areola/nipple to infection (Dr. took me off antibiotics the week after surgery) caused by blistering from cauterization. The right breast formed a huge seroma on the bottom (I had complained that I was way too swollen in that area).

I had a revision done one month after the initial surgery, with replacement of left implant and repositioning of the muscle.

I am extremely unhappy with the result. 500 cc implants were set in. That is way larger than my breasts were. The mass removed at pathology was 400g, that would make about 375cc. I need to wear an elastic bandage to "push" the implants down. I feel like I am trying to squeeze a grapefruit in the peel of a tangerine - and it hurts a lot. I have lot of puckering and divots. A lot of pain as well. My back hurts between the shoulder blades. The LD incisions are around 10-11" long. On the left side where my drain was coming out it was towards the abdomen - there is a numb portion and I cannot even suck in my abdomen, I look like I am 8 months pregnant. I had extensive seromas on my back that the PS never aspired, said they need to go down by themselves.

I am now 7 weeks out of the first surgery and 3 weeks out of the revision - and cannot go back to work until the end of January due to restrictions of weight lifting. Except for now being able to get out of bed or lay down without help, I do not seem to get any better. The LD was not de-nerved and the movement of the breast is totally freaking me out. I can barely do every day chores. I strongly doubt that I will be able to do the work I used to do, ever again. I asked about PT, massaging, I was told "you don't need massaging or physical therapy, just do some stretching".

My chemotherapy date has been pushed further due to all these complications, so it was decided that I will go in for revision after chemo, lymph node and possible radiation will be finished - and that is roughly 5-6 months from now.

I do not trust this plastic surgeon anymore and I would like to find someone else.

I'm so sorry this has happened to you, I hope you can find a surgeon to help you. Once you loose confidence in your surgeon it's time to find another. I hope you can move on from this terrible experience and get the outcome that's right for you.

I'm so sorry that things are not working the way you had hoped. There are doctors out there that can help you. I had my surgery 2007 and although I would like to say that it's 100% it never is. You and I are work in progress. Find a new doctor who will listen and offers a support team. I swear by my physical therpist and massage therpist, they pulled me over the bumps. I have to be honest with you - smoking is not a good thing and messes up the healing process. My doctor warned me at the go that smoking was a no and that it would kill graphs like nipples. I know it's hard but you have to let it go. Accept where you are and work at getting as good a you can. count each improvement as a blessing and you will make it past this nightmare. We are never the same in looks or feelings. But you will learn to work with it. Laugh laugh laugh even when you want to cry. connect with other women and get as much info as you can. i had mine done in oooklahoma city by dr. metcalf, and was very happy until i developed capsular 2 yrs. after surgery... (not his fault) i just wish he put implants under the muscle, thats where mine should be, but he was very informative, checked up on me in the hotel, and even after i returned to texas. he also gave me the name of a doctor in texas i can go to indefinitly for check ups...
