I found Dr. Maercks online while researching procedures to correct my double bubble deformity as a result of my recent sub-muscular breast implant surgery. As I read more about the cold subfasial breast augmentation that Dr. Maercks performs, I also learned his cold subfasial method is not only the ideal solution to correct my double bubble deformity, but also to eliminate the large unnatural gap between my breasts that is so often seen with under the muscle implants as well as eliminate the all too often implant migration down and to the sides also commonly seen with sub-muscular implants.
My surgery was performed a week ago and I am very pleased with my results!
My double bubble is gone and my breasts have the perfect space between them. The choice by Dr. Maercks to use anatomically shaped implants in lieu of my previous round implants gave my appearance a much more natural look and upper pole fullness I was lacking with my previous surgery. Dr. Maercks exceeded my already high expectations!

That's great you were also able to address the spacing between your breasts with your revision. What size implants did you get each time? Congratulations!

Hi Beth, As I mentioned in my review, my first surgery was submuscular and my implants were the Mentor Moderate Profile Round 350 cc's. For my revision surgery I had form stable (Gummy Bear) high profile 395 cc implants placed above the muscle and under the fascia.

Hi Beth, I was very curious about that as well. I had expressed to my doctor that I did not want to increase my size. He assured me that though the cc's were higher in my revision, my breast size would remain the same because the silicone in the gummy bear implants was distributed differently due to their tear drop shape in comparison to the round implants I previously had. So, to answer your question, I have remained the same size and very happy about that.

I didn't realize that not posting photos made my review seem suspicious. This is the first time I have ever submitted a review for anything. I felt compelled to do so because my "plight" has been somewhat treacherous and I finally feel happy after a year and a half of going through breast cancer, partial mastectomy, bad breast reconstruction. I finally have the look I feel I deserve. I will gladly submit photos.

Wow! Your result looks amazing! I nursed all three of my kids for 2 years each and ended up going from a natural 32DD to flat as a pancake after the completion of nursing my last child. I really have always loved the natural shape of my breasts and I've found that my recent under the muscle "dual plane" augmentation distorts my natural shape that I've always taken pride in. I just despise looking down to see and even feel my cleavage being pulled to the sides, as my natural cleavage always touched in the middle. Your result looks fantastic but I Am curious to know if you still feel that pull between your breasts ... Like being pulled towards the outsides of your body while being active or just moving about your day? Also ... What incision location was used in your first submuscular BA? Mine is a periareolar incision. I would rather use that incision sight again to avoid a second location of scarring on my armpits. Which incision was used to place your form stable implants in the subfascial plane? Are you still satisfied with your results? I apologize for being such a huge bag of questions but I failed to do adequate research the first time around and have paid for it ever since (including implants that are causing pain in my armpits from being too wide for my BWD) I completely understand what you mean when saying how miserable you were before having your revision! I just want to make sure I do loads and loads of research before i put thousands into my body and out my family through another surgery with me ... I have to make sure it will give the result I desire. Any input you could give me would be incredibly helpful in my search for the best surgical plan. I have read other doctors on this sight say that the subfascial approach is almost no different than than submammory (overs) and that sagging will be present in several months. Have your girls stayed nice and perky? Feel natural? Again, thank you for any help that you can give me!

Well I just read down further and answered some of my own questions lol! I apologize, I'm still getting used to the navigation of this site. I'm glad that your previous incisions were able to be utilized. I would also have to have my under the muscle implants removed first. Was this a staged procedure for you where you had to wait to heal after removal before proceeding with the cold subfascial? Or was all of this done in one operation?

Hi there! I am happy to help with your research in any way I can:) Let me start off by mentioning that I just reached the seven month mark since my subfasial augmentation surgery and I have no sagging or any real change for that matter. I, like you, always liked the natural shape of my breasts but due to my lumpectomy, I felt compelled to improve the appearance of my breasts with implants since my left breast looked atrophied in comparison to my right one. I am so pleased that I chose this method and from what my doctor explained to me, the fascial layer helps support the implants. I guess it works like a bra. With regards to the incision site, his method is to make the incision under the arm but my surgery was done through my inframammary scar (from my first BA) to avoid a scar in an additional place on my body. I don't know if the incision site can be through the nipple with the subfasial method (you should call my doctor and ask) Now with regard to any pulling or distortion while exercising or moving about, I am so pleased to say I have none whatsoever as I had with my first BA done submuscularly. It was done in one operation and by the way, my recovery was less painful than when I had my first BA submuscularly so that's definitely another "up side" to this procedure :)

Thank you SO very much for replying! All of this info is so very helpful! I'm glad that it can all be done all in one operation! Your results are just beautiful and I'm happy to hear that you are still pleased with them. I just hate the gap between my breasts and how the under the muscle placement prevents my cleavage from having the natural cascading look that it once naturally had. I've also felt that these implants are very heavy in the bottom of my breasts. For someone that has had naturally large breasts, this is a weird sensation to me. I think it may have something to do with the muscle pushing the implant down. I just don't have the fullness on top that I naturally had. I'm excited to be talking to someone that has had this operation done by this particular doctor. It seems that just about every single other doctor has flat out dismissed the cold subfascial placement as a marketing ploy with no real benefits. Maybe it's just less known, less studied ... But that doesn't make it "bad" in my eyes. I just want an alternative to this under the muscle placement and I've been told that I didn't have enough natural breast tissue left after nursing 3 children to camaflouge an over the muscle placement. I guess I will place a call to your surgeon and see if they could be placed through the nipple. If not, I may be willing to accept a scar in my armpit area as long as it wasn't too noticeable. I've heard that the transaxillary incision leaves a very insightly scar in a really visable location - like it can be seen when you wave to someone and are wearing a sundress, etc. thank you again for replying and speaking to me about this! Thanks! Yes they are soft because they're silicone and as I was told, silicone feels more natural than saline. Before my BA, I was a large A cup, or small B (depending on the bra) and so because of the cold subfasial placement, you can feel my natural breast over the implant.

Is there any way that you could add another picture or two of your results so far? How do these anatomical implants act when you attempt to put them in a push up bra? Do they flow and "push up" like your normal cleavage? I've heard that the gummy bear implants are stiffer than round gel implants. How is the feel/behavior of the implants? Do they giggle much lol? You are the only interactive BA patient that this dr has on here at the moment! I started researching and have become weary of this doc due to some bad reviews saying that he doesn't have medical malpractice insurance, that he isn't board certified, etc. The land of plastic surgeons is so confusing! A highly opinionated field where piece of mind is hard for the patient to find. With limited info on this procedure and this doctor, what made you feel confident enough to take the plunge on a procedure that so many other surgeons basically condem?

Hi there! My implants look pretty natural in a push up. Perhaps it's due to the upper pole fullness from this type of implant. ( Gummy Bear implants) As far as movement, I like for my breasts to "giggle" ! It reminds me of my natural breasts and these do in fact jiggle. I think if you're natural breasts giggled, that helps your "giggle" factor with this type of implant. As far as the feel, my husband says they feel pretty darn natural and the feel of them has never been a negative issue. When I began my quest to fix my breasts, I called the American Board of Plastic Surgeons and I was told my doctor was in the process of getting his certification, and although I would have preferred to know he already had the certification, I was comforted by the fact he was in the process. Before I chose this doctor, I had seen other reputable plastic surgeons and they too did not have malpractice insurance and though I can agree it may be reassuring to see that they do carry it, it was more important in my decision to know that my doctor was affiliated with a reputable hospital and he is. (3 in fact, and one of which is where I gave birth to my children) I took the plunge, if you will, due to different factors . Some of which Included pictures of his work, His education, articles written in local magazines about him and his work, the innovativeness of the procedure and the fact it was less invasive than under the muscle implants. It's not easy to make these decisions but so far this procedure has exceeded my expectations and only time will tell if I continue to stay satisfied, but today, I am extremely happy so we will see how this journey evolves! Thank you so much! Yes I had submuscular implants almost three months ago and there is not a morning that I wake up and am not depressed by my own body. It's still sore, my implants are much to wide and flat, they hurt under my armpits. I hate, hate, hate the way they feel and hate even more the way they look. I had another revision expert in my area offer to fix the damage done by the previous surgeon for a really, really good price. I will have to have alloderm (really expensive stuff) to help pad out my tissues now that they have been stretched too wide. He will have to raise my crease back to its normal level and give me a more proper implant for my natural shape. He's a great doctor but I have my heart set on anatomical implants (like you have), and he says he can't use them with the existing pocket. More than anything though, I am 100% sure that I do NOT want anything under my muscles. It looks unnatural, it moves unnaturally and certainly feels unnatural. If I could rip these things out myself, I would. So now I'm certain that I want anatomical implants in a subfacial plane. I will watch to see how yours develop over the next year since it will be about that long before I can afford a revision. In the meantime I think I'd rather have these removed, I'd rather be natural and flat chested again with saggy boobs from all the breastfeeding than constantly depressed with a horrible boob job when I look in the mirror. Hi! Ok so I realized I didn't know how you looked so I saw some of your photos and OMG! You look great !! Your breasts, your body, everything! I'm sorry you're not happy but as an outsider, I can honestly say that you look great ! I know you said you're unhappy and I don't mean to diminish how you feel but while your deciding what route you're going to take ,try experimenting with different bras for looks that would please you. As far as the discomfort, I remember my submuscular BA took up to 6 months before I was not feeling any discomfort (try giving it some more time and try to not despair. ). Before this last surgery, I had gone to a reputable surgeon that wanted to do alloderm for me as well along with a lift ! My "before" breasts looked JUST like your "befores" btw. Don't despair, I'll keep you posted on how mine develops over the next year. Remember, we tend to be a lot harder on ourselves, than others are on us. Take care of yourself :)

You are so kind! I just wanted to say thank you for that comment, it helped turn my day around yesterday when I read it. I had been crying all morning. I just can't wait to get these things out! They are so physically uncomfortable! I actually haven't posted any recent pictures as my breasts have progressed horribly. The implants are so wide that they created stretch marks where I had never had them before, yet didn't have enough projection to fill my skin envelope (from chest wall to nipple). Consequently, when I lay on my side, my skin falls off of my implant and my nipple looks like when my breasts were deflated. They have already slid 5 or more cms from my natural crease and I'm basically wearing my bra on my stomach now, it's so gross. I've had double d's before and they never hung this low. There is absolutely no "perk" to my breast at all. Now that they are descending, my Nipples are much higher than I'd like them to be. They are wide, flat and unfortunately huge to me. Also, we tried to use two different size implants to even a slight asymmetry and it ended up making me look more lopsided. My boobs look like stuck on circles way too low on my chest. I also ended up with a pretty bad animation deformity, when I flex it's so ugly and ridiculous! I just hate everything about submuscular implants. Once I have these out I will never ever have my muscles cut into in any way again. Another awesome part of my result is that I have rippling with scilicone lol! I've been told by another revision specialist that the rippling could be more "folding" because of the large base diameter. But none the less it's stretching and stressing my tissues beyond the natural foot print of my breast. It just sucks knowing that I really should have been given a high profile or form stable implant due to my natural chest and breast measurements ... Oh well. If only I could have known then what I know now. And the $6000 would be nice to have back too lol. Thanks for chatting with me, having a disappointing breast aug result in any way is a miserable hellacious experience. It's nice to see someone have such a nice result after being what you went through. I hope I can have an outcome as awesome as yours :)

I just recently read that wearing push up bras with form stable implants can cause the implant to take shape to the bra. One girl on here said that hers dented in in the bottom and then after one night she would wake up and they looked normal. Another woman said that she was unable to push her form stable implants up with a push up bra because they were basically "stuck" in that position. Some said that when they tried to wear a push up bra that the implant outline edge at the top was really visable and prominent. Has any of this happened to you?

Thanks! Oh my gosh! No question! I recovered in half the time and I was able to lift my arms straight away and I was also able to get out of bed on my own (my husband had to literally push me from behind to get me out of bed after my first BA!) Also keep in mind that my surgeon had to remove my previous submuscular implants (only 6 months old!) close my pockets and reattach my muscle before he could place my implants subfasially. The subfasial method would have been less invasive and thus even less painful if I was having my BA done this way the first time around! My surgeon placed the new implants through my existing scars (infra mammary) because I already had my first BA done that way but his method is actually under the arm where there is a natural crease to camouflage the scar. Are you considering this method ?

I have over the muscle umplants and I've just recently had them replaced. Never had any ptoblems in the 20 yrs I've had implants. My original surgeon didn't luke sub muscular at all due to displacement issues etc. when I'm 50 I'd like to go smaller with a teardrop implant. I really like the idea of subfascial so may travel to get it done. Can you please tell me how much it cost you?
