Generally speaking, a mommy makeover includes a breast augmentation with/without a lift and possibly a tummy tuck. The breast surgery corrects the looseness, sagging, and loss of breast volume that occurs after pregnancy, and the tummy tuck corrects loose, stretched skin of the abdominal wall. Your breast photos show stretching and sagging of your breasts, as well as loss of volume in the upper cleavage area. My opinion is that a lift would be necessary to reshape your breasts and correct the sagging, and an implant would be helpful to create more fullness in the cleavage area. This combination surgery is frequently done and should create a nice improvement for you.
