A deviated nasal septum that causes nasal obstruction needs to be surgically fixed. If it is not causing any obstruction then the only reason to fix it is to help straighten the nose. Straightening the nasal septum is integral in straightening a crooked nose. Whether you should do it or not is a personal choice. If it bothers you then you should. A septoplasty is performed for medical necessity when there is blocking of airflow in the back of the nose by a deviation of the bone or cartilage. This will not change the shape of the nose. A rhinoplasty is performed for cosmetic purposes to straighten the nose, and change the shape. Osteotomies of the nasal bones will be required to straighten the nasal bones. A cartilaginous spreader graft may also be required to be placed underneath concave upper lateral cartilage. Dear Shamsher, You will need a consultation and examination to determine a proper surgical plan. It is difficult to provide much information with just the one photograph. If you just want your deviated septum addressed then this would be a septoplasty. If you have other concerns aesthetically then you would have the combination procedure of septoplasty and rhinoplasty simply called a septorhinoplasty. It is difficult to answer your question with just one picture and without the opportunity to examine you but it does appear that you will need at least a septoplasty and a limited rhinoplasty to straighten your nose. With this approach you will improve both form and function.
