Should i get my 4 premolars extracted? (Photo)

Question

I had my braces off three years ago and results were amazing ,, my teeth were crowded so they used expanders and everything was fine .. But now my lower jaw grew and it led for some sort of a bimaxillary protrusion that i really don't like ,, i took two opinions and they both suggested that i should get 4 premolars extracted along with my wisdom teeth. Will negative side effects such as TMJ and a shrink in profile face occur ?? plzz helppp

I agree with Dr Gorman and would suggest using the DNA Appliance (epigenetic Orthodontics) to widen the mouth and allow the anterios to be positioned bore lingually without blocking airway.The DNA appliance will orthopedically grow bone and pneumopedically grow airway hile creating space for the anteriors. Well, doing ortho to early has struck again.  Since your probably male, you had what is called post pubescent mandibular growth.  The lower jaw grew forward and changed everything.  The problem with removing the premolars is after you have to close the spaces.  When the spaces are closed, with ortho, the lower teeth are moved back, which pushes the tongue back and can cause airway/breathing issues known as obstructive  sleep apnea.  The tongue can and usually does block the airway especially when  sleeping. We treat many people that want us to reopen the bicuspid spaces and place implants to improve breathing issues.
