Surgeon suggested muscle suspension for my lip lift, wouldn't that alter mouth movements?

I had my first consultation for a surgical lip lift. After the doctor examined my lips, he said that my philtrum is stiff, and cutting skin only would put a lot of tension on the scar. However, i want to know if that is a common practice? What is muscle suspension?? And wouldn't that cause deformities to mo.uth movements/ smile Thanks in advance
