PIH at week 3 post my first Infini RF microneedling, I still see dark spots on my cheeks and I am worried!

Question

I am now exactly 3 weeks post my first RF infini microneedling treatment. I have developed some bad patches of PIH (on the cheeks). My plastic surgeon prescribed Pigmanorm but i am worried about the side effects and how long the treatment is going to take (i have to work with customers). Any input is greatly appreciated!

PIH is a process that is quite common after surgery. Known as post-inflammatory hyperpigmentation, PIH can make your scars look far worse than they should be. We can develop PIH after acne scars and after surgery, particularly if you have darker complexions. In clinical practice, we always begin with application of Melarase AM and Melarase PM creams (coupled with Melapads) to help smooth out the color and pigmentation. Once the creams are started, lasers can be used to further reduce the scarring. Best,

PIH can occur following an infini treatment, but not commonly. It occurs more frequently in darker pigmented patients. Time will settle this reaction down. HQ's and a good sunscreen will improve these darkened areas.

I would like to see pictures and know your skin type. Also settings would be helpful. This is a rare phenomenon which I have not seen but heard about. It is caused by prolonged redness/inflammation in darker skin individuals in general. Either the settings were too high, too superficial, or repeated in the same area in too close a period of time. I use Zimmer chiller to help cool the skin during treatments. At this point, hydroquinone is needed to reduce the PIH. Sometimes light laser treatments and chemical peels will help. Prevention with using hydroquinone before and after Infini is a good idea if you have future treatments.

PIH can occur with INFINI, despite what the company says. It only occur if treatments are shallow, or if energy levels are high- long PD in darker skin types. The use of SPF, HQ and time will settle things down. All the best, Dr Davin Lim
