I hate breathing at night also add the tip droopy, the thin narrow nose that's long and on inspiration it conclaves further touching sides. Additionally it flares when I smile and points down... help please!

From a cosmetic standpoint your bridge needs to be lowered snd your tip raised. Hard to know what you insurance would cover but unlikely to cover anything cosmetic.

Rhinoletry and a good nasal speculum and perhaps fiber-optic examination will answer your intranasal breathing issues. Having said that the ultimate procedures for your nose will include both functional and cosmetic septalplasty and Rhinoplasty. Only you and your surgeon and your insurance company can answer whether some of your required surgery will be covered by Medicare insurance.It appears likely that spreader grafts to widen your lower lateral cartilage and open up your nostrils a bit will be required.

Rhinoplasty is performed on the outside of the nose to change the shape of it and is considered cosmetic. To prevent collapse requires placement of some structural cartilage grafts. Both functional and cosmetic nasal surgery can be performed simultaneously under one anesthesia, expect to pay for any changes to the outside of your nose yourself.
