Question

I'm a 41-year-old male. My bottom lip is more prominent on my left than on the right. Would a little lip filler on the right side even things out? And if so, do you think under 0.5ml would be enough? I'm not looking for enhanced lips, just symmetry. Do you think the top lip would need any filler added or just the bottom?

Based on your photo and description, your concern appears to be a mild asymmetry rather than an issue of overall lip volume. In cases like this, a conservative approach is usually the best option.

A small amount of hyaluronic acid filler placed strategically in the right side of the lower lip can often improve balance without making the lips look noticeably fuller. In many patients with a similar degree of asymmetry, less than 0.5 mL is sufficient. In fact, it is common to start with 0.2–0.4 mL and reassess after the filler has settled rather than trying to achieve perfect symmetry in one session.

From what I can appreciate, your upper lip does not necessarily require filler. If the asymmetry is isolated to the lower lip, treating only that area is often enough. Occasionally, a tiny amount in the upper lip may be helpful if there is a subtle imbalance between the lips, but that decision is usually made during treatment after evaluating your facial movements and lip proportions in person.

It's also important to remember that natural facial asymmetry is very common. The goal is improvement rather than absolute perfection, as everyone's lips have slight differences from one side to the other.

For male patients, I generally recommend a very conservative technique that focuses on restoring balance while preserving a masculine appearance and avoiding an overfilled look. Starting with the smallest effective amount allows for a natural result, and additional filler can always be added later if needed.
