In my experience local anesthetic or digital blocks should be used in order to do adequate treatment with multiple passes in a single session, cooling the skin between passes. Tattoo removal with proper settings is painful without adequate local anesthesia and topical ointments are woefully inadequate to provide proper anesthesia for tattoo removal. There should be lightening of the lines with each treatment. If after several treatments there is absolutely no further lightening then the ink may be too deep for the laser to reach.
