Subcision is a simple, relatively inexpensive, nonsurgical method for elevating and smoothing skin indentations of all kinds. Since the procedure depends upon stimulating naturally new, native collagen and elastic fiber synthesis (neocollagenesis and neoelastogenesis), the results tend to be long-lasting to permanent. Fat transfer, on the other hand, is an expensive minimally invasive procedure that requires two steps: fat sucked out of a donor location with a large bore cannula and then reinjected into the recipient site, again with a large bore cannula. Unfortunately, despite the promise of permanent results, much of the fat, separated from its original blood supply, does not last, and that which does retains the properties of the donor site from which it was taken--meaning that if the donor site was the belly and the belly develops midriff bulge with aging, the midriff fat transplanted into the cheek may also develop the bulge with time.
