I picked up my first set of aligners for Invisalign Express 10 treatment yesterday and was confused to see these "buttons". My dentist had never mentioned these. Everything I have read online says these attachments should be directly on my teeth. However, mine are on the outside of the aligners. I called my dentist and the receptionist said that since my teeth are not overly crowded, he did not feel it was necessary to place them directly on my teeth at this time, but may re-evaluate later.

Answer: The plan was formulated to have attachments. Why you don't have them in your teeth is unknown.
In my practice I would say that 75-80% of all invisalign patients in my practice and I have started over 400 cases require at least one refinement, as we use it as a way to tidy things up at the end. Hope this helps

Forget the surgery stuff. Invisalign will expand the upper arch just fine and relieve the crowding. By the way -- you already have a pleasant smile. Watch out for the trap of "perfection"
