I have hollow under eyes along with skin creping. I do not wish to have surgery at this time but would like to improve the skin appearance by other non surgical options. My doctor has suggested 3 sessions of PRF injections to help generate collagen and improve skin quality. Other research I’ve read has said ths will not help much. Can you please give me some insight on what to expect with this procedure?
Answer: Results build slowly, but work! PRF is a wonderful undereye treatment for a couple of reasons. One, it is very safe. Filler in the undereye area carries some risks that PRF does not have. Second, PRF contains growth factors that stimulate collagen that can improve that crepey appearance and somewhat improve the look of dark circles and hollowing. The results build slowly - you may start to see benefit at 2-3 months after your first treatment, then more results compound after subsequent treatments.

Answer: Research supports the use of PRP/PRF under the eyes PRP/PRF has been shown beneficial in improving the infraorbital area under the eyes. PRF is basically PRP which has activated to form a fibrin matrix, Platelet-rich Fibrin. This creates more of a gel than PRP (Platelet-rich plasma), so it can stay in place a bit longer, so the growth factors will be released into the local area over a few days. Concentration of platelets, red blood cells (RBCs), and leukocytes (WBCs) is important. In my experience, 1.5-2x under eyes, approx 300k-400k platelets/µL seems to lead to a good balance between tolerable downtime swelling and results. Speaking of results, it does take time for PRP/PRF to work, the best results are usually visible after several months as the body gradually rebuilds the area. Usually 3-4 treatments, about 3-4 weeks apart are recommended. If you have significant volume loss, filler might be considered as PRP/PRF would be much slower to improve this. However, filler doesn't really improve the skin texture, ie. crepiness. Filler is more unpredictable than PRP, it can sometimes draw water and lead to swelling or lumps. Finally, PRP/PRF under eyes is often a good companion treatment to using microneedling with PRP, also known as the Vampire Facial, for overall facial rejuvenation.
