Orthognathic surgery will have little effect on the position of the hyoid bone. Isolated geniioplasty or chin surgery, would improve the overall neck/jawline angle and diminish the appearance of a prominent hyoid but not alter the position as well. A chin implant or genioplasty surgery would improve the appearance of a weak chin.
I would recommend trying some filler or even a chin implant before ortho surgery. The recovery and risks of both of those is significantly less. If it is purely a cosmetic thing you are treating we have many options.Hope this helps,Best of luck,DrC
