IV sedation is by far the most comfortable way to have any surgical procedure done and certainly a safe procedure when properly administered and monitored. While any procedure can be done with local anesthesia, IV sedation greatly enhances the overall experience of patients. 

A sinus lift and 4 implants is oral surgery, not general dentistry. Your surgeon has your comfort in mind as well as a realistic understanding of what the surgeries entail. The side effect of nausea and vomiting can be addressed with the anesthesiologist. There are alternative sedatives which may not cause this for you.  No medicine, sedation medicine, is without side effects. 
You trust your surgeon to perform your procedures, trust that he/she has your best interest in mind to create the surgical environment for patient comfort as well as surgical success. 

This kind of procedures are lengthy procedures and it requires comfortable environment for both the patient to be relaxed and calm , and for the doctor himself while performing this procedure and not to deal with patient movements and re positioning on the chair specialy when dealing with such comprehensive procedure of sinus lift and 4 implant placement. Throwing up is most of the time due to nausea caused by sedative medication but  this issue can be controlled by many anti-nausea medications that can take care of that. But again it is preferred but not a must.

Raggedyann6,The surgery required to complete a sinus lift and 4 dental implants is no small task.  That being said, There are patients that can get through it without being sedated as long as they have local anesthesia on board.  You may be getting some resistance from your surgeon because they do not want to risk that you will be uncomfortable during the procedure to the point of them having to discontinue and get an undesired result.  This surgery requires the patient to be still.  I encourage the majority of my patients to be sedated because it is not only easier for the patient, but easier for me.  This in turn leads to better clinical outcomes and less stress.  In your particular situation you may want to request oral sedation vs. IV sedation.  A light or moderate oral sedative is unlikely to cause nausea and vomiting.  Especially with the use of an antiemmetic drug.   Also, I encourage my patients with tendencies for nausea to take anti nausea medication before the procedure as well as after.  Your logic about throwing up causing a potential complication with the surgery is definitely founded though.  For such a large procedure it might be helpful to "test" this type of sedation prior to the actual procedure.  Although it could take more time to do this, it could definitely be worth the investment you are making in your mouth.  Hope this is helpful.Warm Regards,

no you don't, and provably you will not have to do lateral wall sinus lift, this can be done with intra crestal approach,so that will make it more simple.and you will have great results.
