33 year old male skin type 3 with mixed acne scars. I recently had an appointment with a nurse at a derm clinic for Infini treatment. She said she will do 5 passes of infini on me. 2 passes at deeper depth (med energy level), 2 passes at medium depth (medium energy), 1 pass at superficial depth (low energy). i am worried because i an darker skin type, and 5 passes could make me hyperpigmented for a long time. How many passes are typically done to treat acne scars with infini? Please please help.
Answer: Infini settings for acne scarring--Intensif microneedling, lasers, fillers, subcision for improvement Infini is a popular treatment, but I prefer Intensif because the needles are non-insulated and gives more robust results. Keep in mind, a combination approach will give the most improvement. These types of scars need a combination of lasers, microneedling/prp, subcision and fillers. It will take a series of treatments and may need some treatments to maintain. I recommend getting a formal evaluation with an acne scarring expert. Best, Dr. Emer

One must be careful with putting too much heat into darker skin types with the Infini. 2 passes for 3 deeps is fairly standard though. I do feel that the treatment appears to be appropriate and you should do well. If there is any concern about hyperpigmentation, consider using hydroquinones as a preventative measure with Retin A. LED therapy after the procedure will help as well. Also using a Zimmer chiller will help cool the skin between passes and make for a safer treatment.

Answer: Acne and Infini Acne patients generally require more passes and more energy. The scarring is harder to treat than in other patients. For this reason, one must be experienced in the settings and amount of passes. Hypopigmentation is generally not a problem. I would suggest talking to your health professional about these factors.

Answer: Depends on many factors including scar type, depth and severity of acne scars. It really depends I do any thing from 2 passes to 5, depending on the above. If needles are kept at a deep depth, with short PD and lower energy levels, PIH is reduced. Pre treat with HQ, I am sure you are in good hands as the treatment is usually dialled in by the dermatologist. In my practice I do the majority of the scar treatment, and my nurse feathers out the results. 2-5 is my standard. This is my preferred treatment method for darker skin types. Video out soon. Acne scar revision is a sub-specialised field. For the best results, one should target the acne scar type with ideal treatments and not just one device. The web reference below will take you to a resource to help you understand acne scar revision at a specialist level. I embedded all the videos in one page to help. For example deep ice pick scars, and narrow box car scars can be treated with TCA CROSS peels, mixed scars, rolling, and atrophic scars treated with fractional devices such as Fraxel, fractional lasers, PRP and INFINI radiofrequency. Atrophic scars (depressions) can be treated with either fat grafts, or with HA dermal fillers. Tethered and anchored scars are best treated with surgical techniques such as subcision. Other surgical techniques that we use include punch elevation, surgical elevation, punch excision and traditional excision of focal scars. The majority of patients will have a collection of different scar types, and hence a tailored treatment METHOD will be best. Careful examination, especially under angled lighting with scar mapping will give you an understanding of what are the best options for your scars.
