I just had the 3rd Infini RF treatment two days ago. The depth settings were at 2.5/2/1.5 mm, three passes each. I am curious to know why she chose to treat the top half of my cheeks just at 2.0 mm and not 2.5 mm, like she did for the bottom part of the cheeks and towards the bottom jawline? Most of my deep scars are on the top part of my cheeks. I wish to go for a 4th treatment I felt that she should have used 2.5 mm all over the cheeks for maximum collagen building. Your thoughts please?
Answer: Needle depth for Infini treatments Thank you for the great question. Infini needle depth can range from 0.5mm to 4mm. The depth chosen for each specific area of the face is determined by the skin thickness. The skin of the mid and lower cheek is typically thicker than the upper cheek (especially as you get close to the eyelids). Treating too deeply would actually be less effective and may cause more bruising or injury to deeper tissues.

Answer: Infini settings for acne scarring--Intensif microneedling, lasers, fillers, subcision for improvement This is difficult to answer without pictures. Infini is a popular treatment, but I prefer Intensif because the needles are non-insulated and give more robust results. For scarring, a combination approach will give the most improvement. I recommend a combination of lasers, microneedling/prp, subcision and fillers. It will take a series of treatments and may need some treatments to maintain. I don't know the reason for this unless the operator felt that the skin wasn't as thick in this area. Also, this is an area of more discomfort and he/she might of felt that more shallow would be more comfortable. Does it make any difference in the results? Probably not, unless the scars are very thick in this area. There is one comment that states that the Intensif is more robust than the Infini because of uninsulated needles. I disagree. The energy must be less with uninsulated needles so as not to cause pigmentation problems or scarring. The Infini can deliver far more energy than most or all RF needling devices because of the insulation. In fact, it can deliver more energy than the skin can tolerate (with my research) if settings are tweaked. If the doctor desires uncoated needles with the Infini, the needles can be set at 0.5mm and get similar effects to the skin as Intensif. This is done at very light settings in light skin for superficial wrinkles and scars, in very select patients.

Answer: Bespoke treatments with INFINI RF There are guidelines for INFINI based upon standard op protocols however every specialist I know will have their own setting based upon your skin type, concerns, level of scarring and skin thickness. I perform all my INFINI setttings myself, the first few shots are 'test' shots - especially if I am treating acne scars. This determines if the solenoid is strong enough for me to goto the desired level - usually 3.5 mm. If I was to target pores, my treatment level would be very superficial- namely 0.5- 1.5 mm and most of the energy delivered there. Bottom line, treatments should be bespoke, and no one should have the same level, energy and pulse duration - as we have skin as individual as we are. I wish you well. Dr Davin Lim
