Incision seemed to open/scab ripped off after removing surgical bra. What should I do? Will it heal or scar? (photo)

I understand your concern and I am sorry to hear about what you are experiencing. However, it is difficult to assess your incision site with mere pictures. It is best that you visit your plastic surgeon to have yourself properly assessed. Only after a thorough examination you will get more information and recommendations.

I'm sorry to hear of your complication of the augmentation mammoplasty procedure. Delayed healing may result from less than optimal blood supply.The cause of this impairment can range from effects of smoking,
diabetes mellitus, vascular disease, previous surgery in the area (disrupting
the normal blood supply), infection, or may totally inapparent.  
Treatment consists of removal of the unhealthy tissue, followed by
intensive wound care. One looks for the development of granulation tissue, a
beefy red vascular tissue, to develop, after which time the wound will begin to
shrink. Most wounds of this size will seal over a span of 1-2 months.
However, a regimen of frequent wound care is required after the unhealthy
tissue is removed. 
Contact your plastic surgeon if he / she is unaware of the problem
and follow instructions given.
The appearance of the final scar is not yet apparent. If it proves unsatisfactory, revision is a possibility in  6-12 months.
