Is it normal for acne to get worse before getting better with Spironolatone? 1 week post op.

Question

I started taking Spironolatone a little over a week ago and have noticed that my face is breaking out more and my pores seem to be larger/more noticeable since starting it. I also got a Mirena IUD about one month ago. I have been on two rounds of Accutane and since that only worked for a few months, I didn't want to go through another cycle of that. Just wondering if this is normal and trying to be patient and stick it out for a few months!

Hello and thank you for your question. Spironolactone when used for acne (off label) can take at least 3 months for effect to take place. Side effects typically do not include acne flare, so there may be other causes for the breakouts. There are many other modalities for acne treatment, including topical medications and aesthetic procedures. It is difficult to adequately assess your skin through this type of forum, and thus I recommend a consultation by a board certified dermatologist for evaluation of your skin and discussion of appropriate treatment. Best of luck
