I had breast augmentation over 2 years ago and I've been told my pockets are too large. It causes pain by my shoulders and my armpits. Is it necessary to fix it right away or will the pockets get any larger if I wait? I have dual plane, 400, 450 cc silicone.

Answer: Large Implant Pocket A large implant pocket is not typically associated with pain. I would recommend visiting with your plastic surgeon and having a detailed examination. If the implant moving around causes you discomfort, then repositioning the implant into a new pocket will help. Pocket revision surgery provides great results by providing control of the implant. Correction (usually involves capsulorraphy surgery) is not urgent but will likely be a good idea sooner rather than later, given your symptoms. It is hard to predict whether the displacement problems will worsen over time or not. My best advice: select your plastic surgeon carefully. Make sure that he/she has significant experience achieving the types of outcomes you will be pleased with. Then, communicate your concerns and goals carefully, as well. I am assuming you see this.Large pockets are not necessarily associated with pain.It is unpredictable if the malposition will get worse with time and there is no urgency regarding correction.I think that doing the surgery for pain relief would have to be very individualized as most patients do not have pain. A careful history and exam would be necessary to predict what the outcome of the surgery would be regarding your symptoms.Since you likely have another reason to undergo pocket revision (malposition), hopefully it will improve your symptoms as well.
