47-year-old male, Indian skin (Fitzpatrick IV–V). Photos taken clean-shaven, no products on. Forehead bumps, present since at least June 2026: - Adapalene 0.1%, alternate nights, about 3 months. Well tolerated, no purge, no clear change. Stopped late September. - Professional steam extraction on 6 Oct: some soft plugs and some hard white "pearls" came out. - Since then: azelaic acid 15% every morning, daily SPF, and no hair wax. Nose: a few flat dark-brown spots, 2–3 mm, on the bridge and sides, present for [X months]. Questions: 1. Do the forehead bumps look like closed comedones, milia, or something else? 2. If milia, is lancing by a clinician the right fix? Does a retinoid help them? 3. Should I restart adapalene, or would tretinoin or something else work better? 4. What are the nose spots, and how would you treat them on darker skin without causing more pigmentation?
