6 months post op, pain concentrated behind my nipple. Fat necrosis in my breasts?

I got fat transfer to my breasts 6 months ago. The pain went away after 2.5 months. At 4 months post op the pain returned. Soreness. Tenderness. From below the armpit to underneath each breast. I reached out to my doc and they said that since it had been 4 months since surgery it was unlikely to be linked to surgery. The pain started to subside so I went about my life. The pain returned and now it’s worse. Concentrated behind my nipple. Almost feels infected. Does this sound like fat necrosis?

If you have enough breast tissue to support the transfer, then fat may help. If you do not, dermal grafts like Alloderm could be placed internally for more fullness.
