Fat transfer is not an ideal treatment for malar bags because the additional fat will create accentuate the puffiness around the eyes. Instead, to minimize the appearance of malar bags, radiofrequency micro-needling (Morpheus8) effectively reduces the fatty eye bags while tightening the skin and promoting collagen growth. This technique can be applied to any skin type and the results are finalized about 6 months later. Additional treatments may be necessary to completely reduce the malar bags. Thank you for your question.If you already have fullness in the malar region, then fat grafting would not be the best treatment for that. Typically a transconj lower bleph is done to remove the fat pads which cause the fullness in that area, and if needed skin tightening is done. I would suggest an in person consultation to determine exactly what your best option is.
Though more complex because of the necessity of fat harvest before injection, it is no more risky than use of off the shelf fillers. Each can give excellent results. Both, however, require an expert injector experienced in treating the under eye area. Make sure you choose such a physician.

You can resume activity after 7-10 days, if you do excessive exercise and lose weight you may lose some of the transfer. at 6 weeks the majority of what should stay would have stayed. Add Sculptra to improve results. Best, Dr. Emer.
