Please consult a cosmetic dermatologist or plastic surgeon who can assess your face and recommend appropriate treatments for you.  Botox and fillers, such as Restylane or Juvederm, can restore a youthful appearance.

First, I would not undergo thread lifting. I don't think it will benefit you and will waste your time and money. Second, it is helpful for you to bring in a photo from an age where you liked the volume in your face. In your thirties, you begin to lose fat in your cheeks and thus will have some room for fillers to improve the contours and re-establish some of the youthful curves you have lost. I would have a discussion with your surgeon about chin implantation. You have a small chin vertically and an implant could improve some of the harmony between the lower, middle and upper third of your face also decreasing your need for revolumization. Finally, when you have decided what your goals are, you should then determine whether you want to use fat, hyaluronic acid, or other types of fillers with your surgeon.

I would start with fillers. 1/2 syringe of Radiesse per cheek bone, 1/2 syringe of Restylane silk per lower lid/tear trough, 1 syringe of Juvederm spread around the mouth region. Then I recommend doing Ultherapy for the upper neck and lower face. 

Dear mp1238mp: Thanks for sharing your photos! I understand your dilemma! While fillers are the way to go for you, they are more temporary and do not diminish the soft tissue descent from the cheek to the jowl. Lipocontouring the lower cheek, jowl and if you need it, the neck with fat grafting the cheeks from the harvested fat may be the answer for you. For well selected patients, Thread Lift has worked beautifully!Please see the link and gallery photos.I hope this helps. Thank you for your questions and for submitting photos.  You have lost some volume in your cheeks which is causing your tear troughs to look hollow, and for your lower face to appear a little lax.  You do not look like you have much laxity for a lift at this time.  An expert injector can augment and sculpt your face using fillers and it will look more youthful, and very natural.  You do not have deep lines caused by muscle movement at this time (between the eyebrows, on the forehead, or on the "crow's feet" area).  However, you could still benefit from Botox.  Botox will eliminate the subtle lines you do have, and will help to maintain a more youthful look if you continue to use it over time.  Good luck!

  Hi, from the photos your face is an elongated oval shape with flat cheeks anteriorly (in the front), a weak chin, jowls and excess facial fat below and in front of the cheek area.  Following the beauty principles outlined in my book on face and body beauty, women look the most feminine and attractive with heart shaped faces.  Heart shaped faces have cheeks that are full and round in the front.  Cheek augmentation with a dermal filler (I prefer Restylane Lyft) or using cheek implants for a permanent enhancement will create full, round cheeks in the front that will feminize the entire face.  I would combine this with facial liposuction to reduce the facial fat and further shape the cheeks.  This should a part of any facelift procedure IMHO.   When the chin is weak, this creates an imbalance in the face making the nose look larger and the mid face appear top heavy.  Chin augmentation, with a chin implant, will add projection to the chin creating harmony and balance to the lower face.  If you have "jowls" this is sagging
facial tissues and an indication for some form of a facelift, not Cheek
Augmentation. I have performed many Cheek Augmentations over the past 30
years and the notion that Cheek Implants or dermal fillers alone to the cheeks
will lift tissues is an urban myth. Filler to the cheeks or cheek
implants will shape the cheeks but will not lift up the jowls. The
underlying SMAS layer, of the face, must be dissected, lifted, trimmed and
re-sutured. The excess skin is then removed and the facelift incisions
closed.
