Plastic surgeons are often called upon to create nipples in cases of breast reconstruction following mastectomy. In those cases, we can definitely create reasonably large nipples. However, they very often tend to flatten out over time, and so this is a very difficult thing to achieve as a long lasting result. Further, reconstruction of the nipple in the case of mastectomy involves rearrangement of the surrounding tissues. In your case, without a preceding mastectomy, the tissues which would be rearranged would be your areola, and so it most likely would not be a very reasonable thing to sacrifice the aesthetics of your areola to try to achieve a larger nipple, particularly in light of the fact that the in large size of the nipple may not last very long.
