i don’t have much body fat but it’s right above my breasts on my chest, it’s already scarred so removal would probably take much longer and still be a scar left anyway, would surgical removal effect my breasts/ make them look odd? it’s about the size of a quarter

As a dermatologist, I would reassure you that a small, quarter-sized surgical tattoo removal on the upper chest typically does not directly affect the breast tissue itself. The breasts sit deeper, composed of glandular and fatty tissue, while tattoo excision is limited to the skin and superficial layers. So from a structural standpoint, it will not “damage” your breasts.

That said, your concern about appearance is very valid. Because the area you’re describing is right above the breasts and you have minimal body fat, the main issue is not the breast itself—but the scar and how the skin heals. Surgical removal replaces the tattoo with a linear scar. In a tight area like the upper chest, that scar can sometimes pull slightly on the surrounding skin as it heals, which in some cases can create a subtle contour change or “tethering” effect.

Since you already have scarring, your skin may be more prone to thicker or more noticeable scar formation, which could make the result more visible than the tattoo itself.

For a tattoo that size, I would usually recommend laser removal first, even if it takes multiple sessions, because it avoids creating a new surgical scar in a cosmetically sensitive area.

If you do consider surgery, a dermatologic or plastic surgeon can plan incision placement carefully to minimize any distortion.
