Regarding buttock/hip augmentation, let me start off by saying that I currently see at least 2 - 3 patients per week that failed "brazilian buttock lift" (i.e. the fat melted away after 10 - 12 months) and are now seeking a more reliable and permanent option (i.e. buttock and/or hip implants). Allow me to share with you some information that you may not hear elsewhere. There are only two proven safe and relatively effective methods for Buttock Augmentation and Hip Augmentation: 1) Autologous Fat Transfer (using your own fat, transferring from one area of the body to the other) and 2) Buttock/Hip Implants (semi-solid silicone rubber implants that cannot rupture &/or leak). Both are options but what it comes down to, like any surgery, is proper patient selection.Indeed because at least 50+ % of the fat transferred will melt away within a year, most patients are not good candidates because they lack an adequate amount of fat to harvest. Another tip is that if you purposely gain weight (i.e. fat) for the procedure, the fat you lose first as you get back to your baseline weight is that very same fat that was transferred into your buttock...so don't fall victim to this recommendation. Although using your own fat is relatively safe, the one serious complication that can rarely (< 1%) happen is "fat embolism" in which some of the fat gets into the blood stream and travels up into the lungs, heart, and/or brain causing serious problems. This complication is more likely to happen with the larger amount of fat being transferred. This is even more likely to happen when using fillers like liquid silicone, PMMA, and hyaluronic acids. Also fillers, when injected in large quantities, have a relatively high infection rate, guaranteed tendency to migrate away from the original area they were injected, and almost always stimulate a lot of inflammation with a subsequent and disastrous amount of scar tissue/hardening.Thus buttock/hip implants become a very good, safe, and long term reliable option for most patients seeking buttock/hip augmentation (at least in my practice). I prefer to insert the buttock implants through a 1 1/2 inch long incision along the inner curve of each upper buttock cheek at the level of the tailbone, similar to two opposing parentheses ")(" (concealed within the crevice between the buttock cheeks) and the hip implants through a ~ 1 inch incision just below the beltline above the hip region. The buttock implant should always be placed under or within the gluteus maximus muscle (NOT "subfascial" as this is no different than subcutaneous or "on top of the muscle"). In this position, the implant is less palpable, less visible, and does not sag or shift/migrate over time unlike implants placed on top of the muscle. Traditionally, teardrop oval implants have a 20+% risk of rotation and thus unnaturally change the buttock shape/appearance when doing so. However i have recently created and patented a new oval shaped implant that avoids this issue. The benefit of this is the fact that ~80% of women patients have more of an oval shaped buttock cheek than round, thus creating a much more natural and harmonious buttock contour. On the other hand, hip implants are placed under just the fascia because no significant muscle exists in this region. However, because they are much smaller and lighter, their likelihood of migration is relatively low. Because all of this, it is extremely important to seek consultation with a board certified plastic surgeon who specializes in this procedure so that the implant placement is precise for both locations. And in this case too, at least in my surgical practice, the infection rate is minimized to <2% (much lower than and contrary to popular belief by many other surgeons not experienced in buttock implants).Depending upon the amount of your current fat reserves and their distribution, you may or may not be a reasonable candidate for fat transfer. If you are wanting to utilize the fat to further augment your buttock in addition to implants, then I caution you against this. The reason being that the fat needs to be placed into the buttock muscles for optimal survival, however, this is exactly where the implant needs to reside and thus risks infection and/or death of the fat. Because of this risk, I frequently avoid fat grafting into a buttock that has an implant. Bottom line, I recommend consulting with a board certified plastic surgeon specialist in buttock augmentation to determine what option is best for you. Glad to help.

The most common method is through the use of fat-grafting, popularized as the "Brazilian Butt Lift", or BBL. This procedure involves performing liposuction of diet and exercise resistant areas of the body (typically the abdomen, waist, hips, and back), followed by processing and purifying the fat before injecting the fat back into the glutes. This fat can be used to recontour the buttock region, while adding volume and firmness. The second method of butt augmentation is through the use of buttock implants, a better option for those with otherwise little body fat. Injections of any other material is at best, unsafe, and at worst, can be dangerous to your health. Please do not entertain this option.Cost of surgery depends on a number of factors, including the type of procedure performed, the anticipated amount of time in the OR based on the amount of fat that will need to be harvested, the area of the country that the procedure is performed, and the facility where the procedure is performed. Price can range from 5K to 20K, with the average in the 10-15K dollar range. Though price is always an important factor, be sure to research your ASPS board certified plastic surgeon for their experience, knowledge, and professionalism. Do not get silicone buttock injections (or any other type of injection that is not your own fat). It is probably illegal, and a "cheap" procedure now will only be more costly (time, money, and the result if you have a complication) in the long term if you get inflammation or infection.Injecting your own fat is an option. Not only do you get fat placed into your buttocks for more volume and shaping purposes, but you get liposuction from your "problem areas" (bonus!). The cost of the procedure is typically dependent on the operative time, facility, and the doctor's professional fees in your area. I would contact a board certified plastic surgeon who has experience with Brazilian Butt Lift fat transfer procedures to discuss your options.
Injections of material such as liquid silicone and other illegal items should be avoided. Not only is it illegal in the US, it's caused deaths. Injection of your own fat is the safer, accepted method of buttock injection, and costs vary between $5000 and $20000 depending on where you get it in the US.
Dear Sharnivia, There are 2 kind of butt injection . One which is illegal in the US - liquid silicone injection and second, which is a waste of your time and your money - fat injection. The fat disappears completely or almost completely within weeks or months as I have seen on many patients who came to my surgery center for butt implants surgery after their big disappointment from the fat injection procedure that was done by other surgeons. Think about it logically : If fat injection was consistent and reliable, most women would have preferred fat injection for breasts augmentation over breasts implants. The reality is that the number one cosmetic surgery in the US is breasts augmentation with implants... Your best option is ultra soft silicone implants. They do not disappear and when done correctly, they appear natural and are very attractive. You have, though, to be careful, because not all results are the same. This is because there are different techniques, different implants , different experiences and different skills. So, do your due diligence and choose wisely. Always consult with experience board certified plastic surgeons who do lots of butt implants surgery in accredited surgery center for your safety. Check the before and after pictures in the photo gallery, to make sure that they are numerous, consistent, attractive and look natural. The cost of butt implants in my practice, for patients who did not have previous butt surgery (fat injection, implants etc.) and requests implants up to 565 cc, is $8500.
