Can I expect the scab from a 4 week tiny skin graft from ear to nose (basal cell) to fall off? (Photo)

Continue to keep the surgical site well-hydrated with vaseline ointment. I would expect the scab to fall off in the next 1-2 weeks. Follow up with your surgeon in order to get the best results.

Full-thickness skin grafts like this are always a work in progress until they finally achieve their final result.  At this point I think the most important thing is to keep it well hydrated with plain Vaseline and keep it covered with a bandage until all the scabbing and and crusting has gone.  Do not worry about any lifting at the edges at this stage.  I would stay in touch with your surgeon because this could well need to have some intralesional steroids or dermabrasion in 6-12 weeks in order to get the best cosmetic result.  All the best.

If the scab is starting to lift at the edges, I would expect it to fall off within the next 1-2 weeks.  Do not attempt to pick or pull it off as that might damage the graft underneath.  I would keep the area hydrated with vaseline.  I would recommend following back up with your surgeon though so that they can make sure you get optimal cosmetic results.
