Dog ear from MOHS. If you do have it removed, is the scar bad looking?

Standing cones or dog ears can result after reconstruction especially if a local flap was used. If it is mild, I would recommend waiting 12 months to allow the skin to redrape and settle. If you have very large standing cone it can be surgically excised. With regard to scarring, this should be similar or better than the original repair. If your Mohs surgeon did a good job with the repair itself, you can likely expect similar results after removal of a dog ear. Dog ears or tricones are excess or redundant skin that forms when a wound is closed. Sometimes they will resolve; sometimes they will not. Dog ears over the forehead and scalp tend to flatten out with time. Dog ears on the nose will flatten out at times on the nose. Steroids generally will not resolve dog ears but will help with excessive scar tissue (hypertrophic scar or keloid). If they don't resolve, best option is to surgically excise the dog ear. As with any surgery, there will be a scar after excising a dog ear so the key is to have confidence that the doctor performing the surgery will do the best job possible. Good luck.
