I am 53 years and in 2009 I had a lower blepharoplasty. I wonder why I still have bags under my eyes even after having this surgery. I do have hashimotos and I am on 30mg of amour thyroid. I am gluten free and have almost reduced my TPO antibodies to 0. Otherwise, I am in great shape. Do you think I need another surgery or am I stuck with these bags.

You need to see your surgeon about this. It could be remnant of a stitch or a tiny cyst which are common along the suture line after eyelid surgery. I'm sure your surgeon wants you to have a smooth recovery and would be happy to see you for this.

From the picture you provided there is no ptosis. The eyelids can be swollen in the morning for many reasons but the most common is simply gravity that redistributes the body water while we are lying down. It may also be related to seasonal allergies. No surgery for now. Good luck.
