Why would I still have bags under my eyes after having a lower blepharoplasty?

I am 53 years and in 2009 I had a lower blepharoplasty. I wonder why I still have bags under my eyes even after having this surgery. I do have hashimotos and I am on 30mg of amour thyroid. I am gluten free and have almost reduced my TPO antibodies to 0. Otherwise, I am in great shape. Do you think I need another surgery or am I stuck with these bags.

Residual eye bags after lower blepharoplasty may be due to excess skin, residual excess fat or hollowing along the orbital rim. Posting photos would help us determine if additional treatments are warranted.

Thank you for your question. You are now 6 years after your surgery so recurrence of your eyelid bags is certainly a possibility. The best comparison is to look at your before and after images both in the early post operative period and more current images. I would review with your plastic surgeon and reassess your options. songbird,Without photos, both now and before and early after your blepharoplasty it is difficult to respond specifically. Generally, the protrusion can be residual, or recurrent, or may be associated with skin laxity rather than fat.  Also, you may have persistent medial tear trough depressions that mimic the shadowing of the lower lid fat protrusion, and those tear troughs are amenable to filler injection, but only medially. Care needs to be taken not to remove too much lower lid fat in order to avoid a sunken or cadaveric look, and sometimes it also is necessary to look at the impact a facelift may have by shortening the lid to cheek junction distance by restoring the cheek fat to proper position below the lids and over the cheekbones.  This response gives a number of general possibilities, but nothing specific can be offered without exam.  I hope that his helps, though.
