Is it OK that dental implant screw is still exposed on inner side after permanent cap has been placed? -Tooth #19 had permanent cap placed May 2017. It felt like food was stuck under it. Days later, asked dentist to check it. Per dentist, exposed implant is fine and that I just need to keep it clean. Is it OK? He mentioned 'not a lot of bone', so I asked why did he do it if not enough bone, after all it cost $2,000-not covered by ins. I didn't get much of a response.

Hi, it appears that your implant is losing bone on the inside area and that the threads are showing. Unfortunately, this makes your implant very difficult to clean and can lead to increased risk of gum infection. I would suggest you see a specialist (periodontist) for a second opinion.

Your outcome is note textbook ideal but can be functional for many years. It is more difficult to maintain implants with the threads exposed. It may be possible to have a periodontist do a soft tissue graft to cover the exposed threads. Fortuantely it is not in an esthetic area.
