I had a hard silicone chin implant done 35 years ago. It has become infected from lower teeth. What should I do?(Photos)

I broke my jaw when I was 6 years of age. It twisted my face to one side. At 35 I had a hard silicone male sized implant put in- I have a big head. I had no problem with it until recently when two of my lower teeth rotted and causing an abscess on either side. Still sore. I don't know what to do. Would removing this be plastic surgery or necessary surgery? It seems to have absorbed the bone. I also have TMJ. What should I do?
