I have been having botox for about 4 years now. I noticed a small bulge above my eyebrow about 2 years ago when I changed injectors and started having higher doses of botox. The lump didn't hurt but would be more noticeable when I smiled. I ignored it as it didn't look too bad and I thought I was just being vain. Recently I've noticed it is quite prominent even after botox so I have waited over 4 months since having botox to allow it to wear off and see if it is the botox that is causing it. However it is still there and doesn't seem to be going. I don't believe it is a cyst but I'm not sure what it could be. I normally have injections every 3-4 months. If anyone could please help I would be really grateful. I've posted a picture of me at rest without raising my eyebrows where you can clearly see the bulge.

Timing and Botox: Botox works by temporarily weakening muscles. A bulge that appears only when smiling or moving could sometimes relate to uneven muscle activity after injections. However, since you’ve waited over 4 months (well beyond Botox’s typical effect), and the bulge persists at rest, it’s less likely that Botox is the direct cause.

Possible causes: Persistent lumps in the brow area can sometimes be due to a few things—fat pad prominence, scar tissue from previous injections, or a small benign growth like a lipoma. Cysts are less likely if it has been stable for years and doesn’t fluctuate in size or become tender.

Observation: The fact that it becomes more noticeable when you smile suggests that muscle movement accentuates it, but the underlying structure is likely anatomical rather than due to injections.

Next steps: While it doesn’t appear urgent or dangerous, it’s helpful to have a provider examine it in person if it changes, grows, or becomes painful. A simple physical assessment can clarify what it is and whether any minor cosmetic or medical intervention is needed.
